VIOLATION)
~ This refers to the inspection
conducted
by M. Hiller, and the inspectors
identified in the enclosed
report, of this office on June
5 through July 23,
1994.
The inspection
included
a review of activities authorized for your
Diablo Canyon Nuclear
Power Plant,
Units
1 and 2, facility.
At the conclusion
of the inspection,
the findings were discussed
with those
members of your
staff identified in the enclosed
report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
Based
on the results of this inspection,
certain licensed activities appeared
to be in violation of NRC requirements,
as specified in the enclosed
Notice of
Violation (Notice).
This violation is of concern
because it involves
a
surveillance test procedure
which was deficient in that it failed to
accomplish
the inservice testing required
by Technical Specifications
during
cold shutdown periods.
As a result, during the last cold shutdown for Unit 2,
four residual
heat
removal
were not properly tested
in accordance
with Section
XI of the
ASHE Code.
Based
on technical
evaluation,
NRC
was obtained
and subsequent
temporary relief from
testing granted until the next Unit 2 cold shutdown or scheduled
refueling
outage.
A noncited violation was also noted involving the failure of a surveillance
procedure to preclude
exceeding
the sampling periodicity described
in the
Technical Specifications for measuring
the effectiveness
of the auxiliary
building ventilation system charcoal
absorber
bed.
Since the criteria of the
NRC Enforcement
Policy were satisfied,
this violation was not cited.

An, additional
noncited violation was noted involving inadequacy
in planning
and performance of an Instrumentation
and Controls surveillance.
The
18C
technician omitted
a step in
a procedure
and, therefore,
failed to remove
an
instrument
from service prior to inserting
a test signal into the control
circuitry.
At a later point in the procedure,
the technician
questioned
the
testing but did not associate
his question with the previously omitted step.
This resulted
in
a test signal
being improperly inserted into control
circuitry, resulting in an undesired
change
in steam generator level.
We are concerned
that two of the events
noted
above involved deficient,
procedures,
which resulted
in failure to comply with Technical Specification
requirements.
Your continuing attention to procedure
adequacy
and
implementation is warranted.
Although
10 CFR 2.201 provides for a written statement
of explanation
to this
office within 20 days of your receipt of this Notice,
we note that this
violation had
been corrected
and those actions,
including your root cause
analysis,
were reviewed during this inspection.
Therefore,
no response
with
respect
to this matter is required.
In accordance
with 10 CFR 2.790 of the
NRC's "Rules of Practice,"
a copy of
this letter
and its enclosures will be placed in the
NRC Public Document
Room.
