Dear Commissioners and Staff:

Pursuant to the special reporting requirements of Diablo Canyon Power Plant Technical Specification (TS) 4.8.1.1.4, and NRC Regulatory Guide (RG) 1.108, Revision 1, PGRE is submitting this special report regarding a failure of Emergency Diesel Generator (EDG) 1-3, to start.

Using the guidance of RG 1.108, Sections B and C. 2. e, this event was evaluated and conservatively determined to be a valid failure. At the time, Unit 1 was in Mode 1. EDG 1-1 and 1-2 remained operable throughout the event and the allowable action time of 7 days was not exceeded.

ln accordance with RG 1.108, Section C. 3. b, the following information is provided:

1.

Diesel generator. involved: EDG 1-3 2.

Number of valid failures in last 100 EDG 1-3 valid tests:

1 3.

Cause of failure: Awire, required for a previous modification in 1993, was not installed in the alternate (nonessential) start circuit. The primary and alternate start circuits are in parallel and are separated (fuses installed), but energized by the same relay.

In the normal configuration (fuses installed), the missing wire did not affect EDG operability or prevent the EDG from starting using the primary starting circuit. However, when the circuits were isolated for testing by removing the primary start circuit fuses, and a start signal was transmitted to the alternate circuit only, the EDG failed to start.

Corrective measures taken: Wiring was added to the alternate start circuit and the EDG was started successfully using the primary and alternate circuits.'.

Length of time the EDG was unavailable: EDG 1-3 was declared inoperable from February 28, 2000, at 2235 PST to February 29, 2000, at 1905 PST, a total of 20 hours2.314815e-4 days <br />0.00556 hours <br />3.306878e-5 weeks <br />7.61e-6 months <br />, 30 minutes.

Current surveillance test interval:

31 days.

7. Confirmation of proper test interval: The total number of valid failures in the last 100 tests for EDG 1-3 is 1. Thus, the 31 day test interval is in compliance with TS Table 4.8-1 and RG 1.108, Section C. 2. d.

This event was not considered risk significant and could not have adversely affected the health and safety of the public.

The enclosed tables provide the number and extent of tubes inspected in each SG and the location and percent through-wall of each indication identified.

Enclosure PG8 E Letter DCL-00-040 Introduction TS 4.4.5.5b requires that the results of steam generator (SG) tube inspections be submitted to the NRC within 12 months following completion of the inspection.

The report shall include:

1.

Number and extent of tubes inspected.

2.

Location and percent of wall thickness penetration for each indication of an imperfection.

3.

Identifications of tubes plugged.

Table 1 provides the number and extent of tubes inspected in each SG.

100% of tubes were full length inspected by bobbin coil, except for rows 1 and 2 U-bends which were inspected by rotating probes.

Table 2 provides the location and percent through-wall of each indication identified, and identifies the tubes that were plugged in the Unit 1 ninth refueling outage (1R9).

Notes:

1.

Free span indications (e.g., manufacturing burnish mark) that had eddy current signals traceable to preservice inspection data are not considered as reportable indications and are not included in this report.

2.

Bobbin distorted support signals that were not confirmed by Plus Point are not reportable indications and are not included in this report. A discussion of bobbin distorted support signals attributed to axial ODSCC are included in the 90 day report for voltage-based alternate repair criteria (ARC), submitted to the NRC in DCL-99-076 dated June 1, 1999.

As requested by NRC's letter requesting supplemental data dated September 20, 1999, and clarification from ERDS Project Officer, Mr. John Jolicoeur, per teleconference on October 7, 1999, this supplement to Special Report 99-05 is submitted to provide additional information regarding data points provided via PGB E Letter DCL-99-086, dated June 18, 1999.

The ERDS system was installed as documented in PGBE Letter No. DCL-91-261, "Emergency Response Data System Implementation Plan," dated October 25, 1991.

The Safety Parameter Display System (SPDS) system was installed in response to NUREG-0737, Action Plan Item I.D.2, and NUREG-0696 (circa 1980).

PG&E upgraded the SPDS computer system during 1996 (reference DCL-98-1 36, "10 CFR 50.59 Report of Facility Changes, Tests, and Experiments for the Report Period May 24, 1996-March 28, 1998"). The upgraded SPDS is Y2Kcompliant and has been further enhanced to provide the ERDS function. PGBE has enhanced the data points requested in NUREG-1394, Revision 1, dated May 1991, with clarification provided by the ERDS Project Officer, as "ERDS Data Point Library, Revision 2," enclosed.

Pursuant to the special reporting requirements of Diablo Canyon Power Plant Technical Specification (TS) 4.8.1.1.4, and NRC Regulatory Guide (RG) 1.108, Revision 1, PGKE is submitting this special report regarding failure of Emergency Diesel Generator (EDG) 2-1 to complete the 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> loaded run due to an

'vercurrent trip.

Using the guidance of RG 1.108, Sections 8 and C.2.e, this event is considered to be a valid failure. At the time of the failure, Unit 2 was in a refueling outage with the core offloaded.

EDG 2-2 remained operable; therefore, the requirements of TS limitingconditions for operation were satisfied.

In accordance with RG 1.108, Section C.3.b, the following information is provided:

Diesel generator involved: EDG 2-1 2.

Number of valid failures in last 100 EDG 2-1 valid tests:

1 3.

Cause of failure: The voltage regulator power transformer failed.

Corrective measures taken: The voltage regulator power transformer was replaced.

Length of time the EDG was unavailable:

EDG 2-1 was not required to be available at the time of the failure. Unit 2 was in a refueling outage with the core in the spent fuel pool. EDG 2-1 had maintenance performed and was being tested prior to being declared operable.

EDG 2-1 was repaired and returned to operable status prior to Unit 2 entering Mode 4 (Hot Shutdown).

Current surveillance test interval:

31 days 7.

Confirmation of proper test interval: This is the first failure of EDG 2-1 in the last 100 valid,tests.

Per TS Table 4.8-1, the test interval should be 31 days.

This event was not considered risk significant and could not have adversely affected the health and safety of the public.
