APPENDIX A NOTICE OF VIOLATION Pacific Gas 8 Electric Company Diablo Canyon Nuclear Power Plant Diablo Canyon Power Plant Docket Nos.

50-275 50-323 License Nos.

DPR-80 DPR-82 During an NRC inspection conducted on April 11-15,

1994, a violation of NRC requirements was identified.

In accordance with the General Statement of Policy and Procedure for NRC Enforcement Actions, "10 CFR Part 2, Appendix C, The violations is listed below:

10 CFR Part 20. 1902(b),

"Posting Requirements,"

requires licensee to post each high radiation area with a conspicuous sign or signs bearing the radiation symbol and the words "CAUTION, HIGH RADIATION AREA" or "DANGER, HIGH RADIATION AREA."

As defined in 10 CFR Part 20.1003, "Definitions," high radiation area means an area, accessible to individuals, in which radiation levels could result in an individual receiving a dose equivalent in excess of 0. 1 rem (1mSv) in 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> at 30 centimeters (cm) from the radiation source or from any surface that the radiation penetrates.

Contrary to the above, between the hours of approximately 4 a.m.

and 4 p.m.

on April 11,

1994, the intensity of radiation at 30 cm from the Unit 1 Liquid Holdup Tank header on the 100-foot level of the Auxiliary Building ranged from 1000 mr/hr to 1500 mr/hr and the area was not posted as a high radiation area.

This is a Severity Level IV Violation (Supplement IV).

Pursuant to the provisions of 10 CFR Part 2.201, Pacific Gas

& Electric Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region IV, and a copy to the NRC Resident inspector at the facility that is the subject of this Notice of Violation (Notice), within 30 days of the date of the letter transmitting this Notice.

This reply should be clearly marked as a '-'Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation or, if contested, the basis for disputing the violation, (2) the corrective steps that will be taken and the results

achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken.

Where good cause is shown, consideration wi',1 be given for extending the response time.
