This refers to the routine inspection
conducted
by D. Kirsch,
H. Hiller and

Gee during the period from April 13 through
Hay 24,
1993.
This inspection
examined your activities
as authorized
by
NRC License
Nos.
and
At the conclusion of the inspection,
the inspectors
discussed
their findings
with members of the
PGSE staff.
Areas examined during this inspection
are described
in the enclosed
inspection
report.
Within these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
The enclosed
report discusses
an area of concern regarding
two inadvertent
boron dilution events
which indicated
a weakness
in your operational
controls.
We note that prompt operator
response
minimized the power increase
which
resulted
from the second
event;
however,
we are concerned
that your admini-
strative procedures
were not adequate
to prevent these
inadvertent dilutions.
Our review of these
events is continuing,
so that
we may fully understand
the
root causes
and programmatic
issues
associated
with these
occurrences.
In accordance
with 10 CFR 2.790(a),
a copy of this letter
and the enclosure
will be 'placed in the
NRC Public Document
Room.
