REPORT 50-275,
50-323/92-27
This refers to the routine inspection
conducted
by Hessrs.
C. Clark and
C.
Hyers during the period from September
14 through October
8,
1992.
This
inspection
examined your activities
as authorized
by NRC License
Nos.
and DPR-82.
At the conclusion of the inspection,
the inspectors
discussed
their findings with members of the
PG&E staff.
Areas examined during this inspection
are described
in the enclosed, inspection
report.
Mithin these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with
personnel,
and observations
by the inspectors.
Based
on the results of this inspection, it appeared
that certain of your
activities were not conducted in full compliance with NRC requirements,
as set
forth in the enclosed
Notice of Violation (Notice).
'Ihe violations described:
~
a failure to maintain the certification of an Inservice Inspection
~ (ISI) examiner for visual acuity,
and
~
a failure to document
and evaluate
the consequences
of measuring
the
vibration of the auxiliary feedwater
pump at
a location other than
that prescribed
by the
ASHE Code.
The violation for failure to maintain the certification of an ISI examiner for
visual acuity is
a repeat violation.
The previous violation was described
in
NRC Inspection Report 50-323/83-04.
Escalated
enforcement
is not applicable
due to the age of the previous violation and the low safety significance in
'this particu'1ar
case.
However, your attention is directed to this and the
several
other indications of a lack of rigor in testing described
in the
enclosed report.
For example,
the report describes
a lack of testing rigor by
corrosion engineers
performing pipe, wall thickness
measurements
at incorrect
locations,
and by surveillance test personnel
measuring auxiliary feedwater
pump vibration at
incorrect locations.

You are required to respond to this letter and should follow the instructions
specified in the enclosed
Notice when preparing your response.
In your
response,
you should document the specific actions taken
and any additional
actions
y'ou plan to prevent recurrence.
After reviewing your response
to this
Notice, including your proposed corrective actions.and
the results of future
inspections,
the
NRC will determine whether further
NRC enforcement action is
,
necessary
to ensure
compliance with NRC regulatory'equirements.
In accordance
with 10 CFR 2.790(a),
a copy of this letter, the enclosures,
and
your response will be placed in the
NRC Public Document
Room.
This letter and its enclosures
are not subject to the clearance
procedures
of
the Office of Management
and Budget
as required
by the Paperwork Reduction Act
of 1980,
