This refers to the routine inspection

conducted

by H. Wong,

M. Miller, W. Ang,

and

B. Olson during the period of September

2 through October 9, 1991.

This.-

inspection

examined your activities

as authorized

by

NRC License

Nos.

DPR-80

and

DPR-82.

At the conclusion of the inspection,

discussions

of their

findings were held

on October 9,

1991, with Mr. J.

D. Townsend

and other

members

of your staff.

Areas

examined

during this inspection

are described

in the enclosed

inspection

report.

Within these

areas,

the inspection consisted of selective

examinations

of procedures

and representative

records,

interviews with

personnel,

and observations

by the inspectors.

Two non-cited violations of

NRC requirements

were identified

as

a result of

this inspection.

These non-cited violations

and four other events

described

in paragraph

5 of this inspection report were caused

in part by personnel

errors which were not detected

by current Diablo Canyon verification methods.

The number of errors

which have occured in the past,two

months

appears

high,

even considering

the ongoing refueling outage

in Unit 2.

Current guality

Assur'ance

trending mechanisms

do not appear to provide timely feedback

on

these

types of developing trends.

The issue of personnel

errors

had also

been

raised in General Office Nuclear Plant Review and Audit Committee

(GONPRAC)

meetings

in July and September

1991.

While some managers

have initiated

corrective actions in response

to these

most recent

personnel

error events,

an

overall site strategy to address

this problem has not been established.

Additional management

attention

appears

warranted to effectively address

this

matter.
