MANAGEMENT MEETING
This refers to the management
meeting held with yourself and members of your
staff at the Region
V office on May 28,
1991.
Subjects
discussed
during the
meeting are sumarized
in the enclosed
meeting report.
Diablo Canyon
has experienced
a number of main condenser
steam
dump valve
failures.
Your corrective
actions for these
valves
now appear satisfactory.
However, your root cause determination
and safety evaluation of the
dump
valve failures did not appear to get the comprehensive
attention they
deserved until after the
May 17,
1991 valve failures.
Now that the valve
internals
on Unit
1 have
been
changed
out with internals of a different
design
we recommend that you remain alert to assure
yourselves
that this
change is
an effective solution to your steam
dump valve problems.
The
PGSE Event Investigation
Team investigation into the Unit 1 loss of
off-site power event appears
to have
been thorough.
In implementing corrective
actions
as
a result of the event,
we encourage
you to consider the role of
supervisors
and management
in the planning
and oversight of maintenance
activities.
In accordance
with 10 CFR 2.790(a),
a copy of this letter and the enclosures
will be placed in the
NRC Public Document
Room.
