NRC AUGMENTED INSPECTION OF DIABLO CANYON UNIT 1

This refers to the Augmented Inspection

Team (AIT) inspection

conducted

by

Messrs.

P. Morrill, B. Olson,

D. Fischer,

W. Lyon, S.

Mazumdar

and

K. Johnston

during the period of March 8,

1991 through March 13,

1991.

This inspection

examined the loss of off-site power event which occurred

on March 7, 1991,

and

other activities authorized

by NRC License

No. DPR-80.

At the conclusion of

the inspection,

discussions

of their findings were held with Mr. J.

D.

Townsend,

and other members of your staff.

Areas

examined

during this inspection

are described

in the enclosed

inspection

report.

Within these

areas,

the inspection consisted of selective

examinations

of 'procedures

and representative

records,

interviews with

personnel,

and.observations

by the inspectors.

Our inspection

concluded that it was providential that no one was killed by

the 500

KV short to ground.

Fortuitously, the air arc causing

the loss of

off-site power occurred

when

no one

was in the electrical

ground path for the

crane. or its load.

In addition, the uncontrolled

use of similar cranes

around'he

500

KV lines appears

to have occurred in the past,

thereby

violating

PG&E Accident Prevention

Rules

and ignoring grave personal

danger.

This situation demonstrates

a disregard for industrial safety.

Despite your staff's review of the lessons

learned

from the Vogtle loss of

power event,

the key issue of controlling vehicle traffic around off-site

power supplies

was not addressed.

The

PG&E strategy left the plant off-site

AC power supplies

vulnerable to avoidable challenges.

Choosing not to deal

with the Vogtle event initiator demonstrates

that your event review process

needs

to be strengthened.

It is particularly discouraging to me to note that just one month earlier the

NRC Deputy Executive Director for Operations

and myself questioned

your

senior site management

regarding Vogtle lessons

learned

and vehicle access

controls

around plaIIt off-site power supplies.

We were assured

that the

Vogtle event

had been reviewed

and that there

was

no problem at Diablo Canyon.

Conversely,

the team considered

your policy to avoid mid-loop operation

when

possible. during outages

to have

had

a positive safety effect and to have

actually simplified the outage activities.

The inspectors

observed that your

schedule

has

been constructed

so that mid-loop'perations

would only be used

if needed at the

end of an outage

when decay heat is low.

Loss of off-site

power or loss of

RHR is much less significant with this strategy.

The issues

raised

by the. AIT in the enclosed

report will be discussed

in

future Management

Meetings.

Any appropriate

enforcement

actions will be

contained in subsequent

inspection reports.

In accordance

with 10 CFR 2.790 (a),

a copy of this letter and the enclosures

will,be placed in the

NRC Public Document

Room.
