D. Townsen

and other members of your staff during the inspection periods

and

conclusion of the inspection.

Areas

examined during thss

Within these

areas,

the inspection consisted of selective

examinations of procedures

and quality records,

interviews with personnel,

field inspections of facilities, and observations

by the inspectors.

The results of this inspection indicate

a continuing weakness

in the

implementation of your quality programs for control of measuring

and test

equipment in ttIe area 0f mechanical

maintenance.

We acknowledge

your recent

effort to improve the timeliness of event-related

corrective actions.

These

actions

were discussed

at length during our management

meeting

on* October 10,

1990.

However,

we are concerned that your problem identification and

corrective action measures

%ave apparently not been very effective in dealing

with the programmatic

weakness

identified during this inspection.

Your

corrective actions in dealing with these

problems

were not considered

comprehensive

nor timely.

Your audits identified problems in this area

as

early as

1985

and

a nonconformance

report written to resolve the problems

appears

to have

been inappropriately closed, without verifying either the

accomplishment or effectiveness

of the cor rective actions .

Your staff appear s

to have

been reluctant to use the formal nonconformance

system to address

the

central

issue of ineffective corrective action, or the issue of the safety

significance of persistent

weakness

in control of MIITE.

As a result of your

staff's missed opportunity to correct

MINUTE problems in the mechanical

maintenance

area

and preclude recurrence,

our inspectors identified what

'ppear

to be continuing examples of prograIanatic

MSTE deficiencies.

Although many of the findings have involved documentation deficiencies,

additional inspection effort is planned to assess

the technical

safety

significance.

Enforcement action related to inspection of this area will be

the subject of separate

correspondence, if appropriate.

In accordance

with 10 CFR 2.790(a),

a copy of this letter and the. enclosure

will b'e placed in the

NRC Public Document

Room.
