HCAP-12064, "Diablo Canyon 1 and 2 Evaluation for Tube Vibration Induced Fatigue," was submitted to the NRC on November 30, 1988 (PG8E Letter DCL-88-290), in response to NRC Bulletin 88-02. As requested by Emmett L. Murphy of the NRC Staff in August 1989, this letter provides additional information to supplement the Staff's review of the HCAP.

During the NRC Staff's review of the Diablo Canyon tube fatigue evaluation, questions arose concerning the effect of penetration of the antivibration bars (AVBs) into row 7 on the flow peaking for tube R9C60 in steam generator (SG) l-l and tube R9C35 in SG 1-3. In response to these questions, a complete review of the AVB locations in Diablo Canyon Units 1 and 2 was conducted. This review resulted in corrections to the AVB maps and a reevaluation of flow peaking and tube fatigue susceptibility at a few tube locations. The reevaluation disclosed that tube R9C60 in SG 1-1 required corrective action. As discussed with the NRC Staff, the deci sion was made to install a cable damper stabilizer and solid plug in this tube. This installation was made during the Unit 1 third refueling outage (1R3) in the fall of 1989. All other tubes reevaluated in the study were found to meet acceptance criteria. Nonetheless, a sentinel plug was installed in tube R9C35 in SG 1-3 as a conservative measure during ID 1R3.
