June 30, 1982 DIABLO CANYON NUCLEAR POWER PLANT, UNIT NO.
1 DESIGN VEPIFICATION PROGRAM A license for fuel loading and lou power testing at the Diablo Canyon Nuclear Power Plant, Unit No. 1, located near San Luis Obispo, California, was issued to the Pacific Gas
& Electric Company (PG&E) on September 22, 1981.
Subsequently, an error in the seismic design of structures and piping in the containment was detected by PG&E and reported to the NRC.
Upon further study by PG&E and consultants, additional errors were found.
The errors raised doubt regarding the adequacy of certain structures, systems and components important to safety with regard to their ability to withstand the effects of postulated earthquakes.
Examples of the errors found include:
the use of outdated seismic response
- spectra, improp r installation of valves (vertically as opposed to horizontal), underestimate of weights in analyses, veriations between actual plant construction and design drawings
- and, the misapplication of vertical seismic floor response spectra used to design piping systems.
Based upon information supplied by PG&E and from NRC staff inspections, the NRC staff identified serious weaknesses.
in PG&E's quality assurance program.
On November.19, 1981, the NRC issued an Order suspending the low power license
- and setting forth requirements for a design verification program that had to be completed prior to restoration of the license for fuel loading and low power testing.
This is referred to as Phase I of the Diablo Canyon Design Verification Program.
In addition, on November 19, 1981, the NRC informed PG&E of additional design verification program requirements that must be met prior to the issuance of a full power license for Diablo Canyon, Unit No. l.
The activities related to that part of the program dealing with the full power license are referred to as Phase IZ.
A number of activities are presently being conducted by PG&E and its contractors for an independent design verification program.
On harch 19, 1982, the NRC approved Teledyne Engineering Service of Haltham, Hassachusetts, as the company to independently conduct the design verifjcation program.
Other participants in the independent verification program are R. L. Cloud, 'Associates, and R. F.
Reedy, Inc., who had performed evaluations of associated Diablo Canyon
'esign and quality assurance matters prior to the staff approval of Teledyne.

June 30, 1982 On March 22, 1982, PG&E announced that H had contracted u(i,th Bechtel Power Corporation to act as the PG&E project completion manager for Diahlo Canyon, Unit Nos.
1 and 2.
The objective of the organization is to ful.ill all requirements for reinstating the low power license for Unit 1
and for meeting all full power license requirements for both units.
A team of senior PG&E and Bechtel managers is dedicated full time to the project with a Bechtel manager responsible for the overall direction.
A Bechtel professional staff of about 80 individuals augments a
PG8E staff of about 120 individuals to form a single, totally integrated PG&E organization for the Diablo Canyon Project.
'On June 18, 1982, PG&E submitted a quality Assurance Program for the Diablo Canyon Project that describes the manner in which the integrated project team of PG&E and Bechtel would fulfillthe equality Assurance functions.
This program is under staff review.
At a meeting on June 10, 1982, in Maltham, Massachusetts, PGKE presented the following schedule for the Diablo Canyon Project:
Completion of Phase I early August 1982 Reinstatement of LP License - end of August 1982 Completion of Phase II - mid October 1982 Issuance of FP Licens,e - end of November 1982
'The staff views the schedule as. optimistic.
The additional verification and sampling, the quality assurance
- findings, and the results of other completed activities to be presented by Teledyne in the near future in interim technical reports must be considered with respect to.their generic implications.
- Finally, a technical report must be prepared by PG&E in accordance ruth the HRC Order and must be reviewed by the staff before the reinstatement of the fuel loading and low power license can be considered.
