0 NOTXCE OF VIOLATION Pacific Gas and Electric Company P. 0.

Box 7442 San Francisco, California 94106 As a result of the inspection of October 2, through October 30, 1981, and in accordance with the Interim Enforcement Policy, 45 FR.

66754 (October 7, 1980),

the following violation was identified:

Facility Opex'sting License No. DPR-76, Section 2.C,(Z), incorporates the Technical, Specifications into the license.

Technical Specification 6.8 states Chat:

"written procedures shall be established, implemented and maintained covering..;applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2,'February 1978...Each pro<<

cedure...and changes thereto, shall be reviewed by PSRC and approved by the Plant.'anager prior to implementation and reviewed periodically as set forth in administrative procedures."

Fux'thex, "Maintenance that can affect the performance of safety-related equipment should be properly preplanned..."

per'section 9.A of Appendix "A" Co Regulatory Guide 1.33. Appendix 4 Section 1.A.l to Nuclear Plant Administrative Procedure E-4 Revision 3, details that:

"Written procedures shall be provided for removal or replacement of faulty equipment having nuclear safety significance when the plant or s'ystcm is in operation.

That is, removal or replacement of equipment which; if incorrectly accomplished, could...activate engineered safeguards

,...or affect the proper functioning of engineered safeguards

'features be performed in accordance with written procedures."

This procedure also deals with the use of sub-tier'procedures, such as>

shop followers," and in Section 6 states that:

"Shop followers shall not be used in lieu of,a detailed written procedure...when the gob complexity is such that an approved detailed procedUre is warranted."

Contxary to the above, there was no adequately reviewed and appxoved procedure for repair and replacement work on-the solid. state protect'ion

system, and as a result, an inadvertant safety injection signal was experienced on October 5, 1981, due to the use of a shop follower in lieu of a detailed, written, PSRC reviewed, plant manager approved procedure, This is severity level V violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Pacific Gas and Electric Company is hereby required to submit to this office within twenty-five (25) days of the date of this Notice, a written statement or explanation in reply, including:
