CPPR-39 CPPR-69 Notice of Violation Based on the results of an NRC site inspection conducted during the period of July 23 through July 26, 1979, it appears that one of your activities was not conducted in full compliance with Construction Permits Nos.
CPPR-39 and CPPR-69, as indicated below.

10 CFR 50, Appendix B. Criterion V, as implemented by Section
- 17. 1. 5 of the FSAR and the PGSE guality Assurance Manual Section V, states in part that, "Activities affecting quality shall be pre-scribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."
Specification No.
8833R-1 (Diablo Canyon Rupture Restraint General Repair Procedure) requires that repairs conform to AWS D l. 1-79 (Structural Welding Code).
Pullman-Kellogg Procedure ESD-243 and AWS D 1.1 require that "Arc strikes outside the area of permanent

welds should be avoided on any base metal.
Cracks or blemishes resulting from arc strikes shall be ground to a smooth contour and checked to insure soundness."
Contrary to the above requirements, the following conditions were observed on July 24, 1974 on the Unit 1 pipeway supports adjacent to field welds that had been inspected and accepted by the contractor:
1.
Six single spot arc strikes and one arc strike about 3/4-inch long existed on base metal below FW-63 of the upper box on restraint bent 4B.
2.
One arc strike about 3/4-inch long existed on base metal near FW-65 of the lower box of restraint bent 4B.
3.
One arc strike about 3/4-inch long existed in base metal of a steel member above FW-78 of restraint bent 6B.
4.
Two arc strikes existed in base metal above FW-84 on the lower box of restraint bent 6B.
This is a deficiency.
