Executive Summary I

I On September 15 and 16, 2016, out of sequence drills (Medical Services and Emergency Worker Monitoring and Decontamination Station) were conducted forthe Waterford 3 Steam Electric Station (Waterford 3), located near Taft, St. Charles Parish, Louisiana. Personnel from I

the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA) Region VI, evaluated all activities. The purpose of the drills was to assess the I

level of preparedness oflocal responders to react to a simulated radiol9gical emergency at Waterford 3. The previous exercise at this site was a Hostile Action Based exercise conducted I on June 24, 2015.

Personnel from the State of Louisiana, Waterford 3, West Jefferson Medical Center, St. Charles I

Parish Hospital Emergency Medical Services, KennerFire Department, and Kenner Police Department participated in the drills. Evaluation areas demonstrated included: Emergency I

Operations/Facilities, Protective Action Implementation, and Support Operations/Facilities.

Cooperation and teamwork of all the participants was evident during the drills and DHS/FEMA I wishes to acknowledge these efforts.

This report contains the final evaluation of the out of sequence drills. The participants I

demonstrated knowledge of their emergency response plans and procedures and adequately demonstrated them. There were no Level 1 Findings identified. Four Level 2 Findings were I

observed and corrected on the spot. In addition, one Plan Issue was identified as a result of the drills. I I

I Unclassified Radiological Emergency Preparedness Program I After Action Report/Improvement Plan 2016 Waterford 3 Steam Electric Station I Section 2: Exercise Design Summary I 2.1 Exercise Purpose and Design The DHS/FEMA Region VI Office evaluated drills conducted on September 15 and I 16, 2016 were to assess the capabilities of local emergency preparedness organizations in implementing their Radiological Emergency Response Plans and Procedures I to. protect the public health and safety during a radiological emergency involving Waterford 3. The purpose of this report is to present the results and findings on I the performance of the offsite response organizations during a simulated radiological emergency.

I 2.2 Exercise Objectives, Capabilities, and Activit~es I Exercise objectives and identified Capabilities/REP Criteria selected to be exercised are discussed in the Exercise Plan, Appendix D.

I I 2.3 Scenario Summary I The drill scenarios were developed to evaluate the response of drill participants to an incident requiring evacuation of the public from the Emergency Planning Zone surrounding Waterford 3. The scenarios provided for an out of sequence Emergency I Worker Monitoring and Decontamination Station drill and an out of sequence Medical Services drill.

I The Medical Services drill was developed to evaluate the response of drill participants I to an* incident at Waterford 3 requiring the transportation, treatment, and d.econtamination of a radiologically contaminated injured individual. The medical I services drill scenario provided for the evaluation of West Jefferson Medical Center and St. Charles Parish Hospital Emergency Medical Services.

I Additionally, a scenario was provided for evaluation of the Kenner Fire Department's Emergency Worker Monitoring and Decontamination Station's ability to conduct I

monitoring, decontamination, and registration for emergency workers and their I'

I I Section 3: Analysis of Capabilities 3.1 Exercise Evaluation and Results I,

This section contains the results and findings of the evaluation of all jurisdictions and functional entities that participated in the September 15 and 16, 2016 evaluated drills to test the offsite I

emergency response capabilities of local governments in the Emergency Planning Zone surrounding Waterford 3. I Each jurisdiction and functional entity was evaluated based on their demonstration of Capabilities and their equivalent REP criteria as delineated in the FEMA REP Program Manual dated January 2016.

I Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart. OROs maintain appropriate record-keeping of the administration of KI to emergency workers. I CONDITION: Emergency workers were unsure of how to tum on the Dosicard Electronic Alarming Dosimeter (EAD) issued during the drill. They had been trained on the use of a pencil I dosimeter prior to the drill.

POSSIBLE CAUSE: Introducing new dosimetry equipment into the field without prior training for emergency workers.

I CONDITION: At the Vehicle Monitoring Station, via inject (Cue Card 2B), contamination was found on the driver's side front wheel. The EW reported the contamination but did not take note I that the survey meter had pegged out and did not change the scale from x 0.1 to x 1.

POSSIBLE CAUSE: Insufficient training on survey meter use.

EFFECT: If survey meters are on the wrong scale, inaccurate readings could be interpreted as nq I contamination, placing a dirty vehicle in a clean vehicle lot and potentially spreading contamination.

I CORRECTIVE ACTION DEMONSTRATION: A timeout was called and training conducted, and EWs demonstrated changing the meter scale accordingly and reported accurate readings.

CRITERION: The facility/ORO has adequate procedures and resources to accomplish monitoring and decontamination of emergency workers and their equipment and vehicles.

I CONDITION: Members of the Decontamination Team were unable to determine correct background readings. I POSSIBLE CAUSE: The Monitor performed the mathematical calculations on a differing scale incorrectly. I

CONDITION: After surveying the contaminated EW, the Personnel Monitoring Recorder I

placed the Personnel Contamination Report Form into the same plastic bag that held the EW's personal belongings. I POSSIBLE CAUSE: Insufficient training on contamination control.

Therefore, 44 CFR Part 350 approval of the offsite radiological emergency response plans and preparedness for the State of I Louisiana site-specific to Waterford 3 Steam Electric Station will remain in effect.

. Waterford 3 Steam Electric Station, in the event of a radiological emergency, Waterford 3 periodically conducts drills jointly with the Federal, State and local I agencies.

This manual contains the scenario of activities and supporting data describing I the Waterford 3 Emergency Preparedness Medical Services, and Emergency Worker Monitoring and Decontamination Station drills.

This scenario was developed by postulating events .that will require response by I offsite organizations.

I, During this drill, the support organizations will respond to the simulated events and conditions that have been selected to provide the level of activity necessary to meet the scenario objectives.

I City of Kenner Fire Department, West Jefferson Medical Center, and St Charles Parish Ambulance will participate in this drill.

I Drill participants will have no prior knowledge of the sequence of events. The drill scenario will allow those individuals and agencies assigned to respond during a radiological emergency to demonstrate their performance according to I current emergency preparedness plans and procedures.

I The scenario is a mechanism by which selected drill controllers will initiate and evaluate the activities of the drill participants. The individual drills will be initiated as shown below and each are expected to last approximately three hours.

I Participating organizations are shown in Section IV, Guidelines, of this manual.

GUIDELINES To define the scope and extent of participation by organizations and participants, I the following guidelines are established in order to meet the objectives listed in Section 111 of this manual.

1. The offsite drills will be conducted as follows:

Decontamination Station Participation is limited to St Charles Parish Ambulance Service Staff, West Jefferson Medical Center Emergency Room Staff, and Kenner Fire I Department and the Drill Control Team. No Waterford 3 plant staff will participate.

(A)V. NARRATIVE

SUMMARY

An evacuee is injured in a motor vehicle accident and has been transported by relatives to the West Bank Bridge Park. The evacuee has been surveyed by I

members of the Parish staff and has been found to be contaminated. St Charles Ambulance Service was requested to provide treatment and transportation of a contaminated patient to the West Jefferson Medical Center for treatment.

11 The Drill Control Team simulates first aid. I At approximately 07:45, the St Charles Ambulance Service is (simulated) dispatched to the patient location. I*

At 08:00, the Drill Control Team calls the West Jefferson Medical Center to report the simulated medical emergency. The initial advisement to the

I ambulance has been that there is an injured individual located at the West Bank Bridge Park who is radiologically contaminated due to a radiological release from Waterford-3. I Once the ambulance arrives, the individual should be medically evaluated. A turnover (from the controller) of the radiological status will be provided to the I ambulance personnel. As medical treatment and patient transport continue, contamination control measures should proceed. The ambulance personnel should communicate with WJMC upon their departure from the simulated I

- West Bank Bridge. Park which should include medical and radiological information as well as an ETA (estimated time of arrival). ~I The EMTs will provide radio updates to WJMC during simulated transportation of the patient. I At approximately 08:30, the ambulance will arrive at WJMC. The patient will be taken into the REA, decontaminated and treated for the simulated injuries. After the patient has been removed from the REA and the proper removal of I

protective clothing has been demonstrated, the medical drill will be terminated.

A critique of the events will be conducted. I Any required x-rays or required samples may be simulated by hospital personnel after demonstrating knowledge of requirements and processes. I I
