Enclosed is the Final After Action Report/Improvement Plan for the LaSalle County Static;m Radiological Emergency Preparedness Partial Participation Plume Exposure Pathway Exercise h,eld on July 20, 2016. There were no Level l findings identified for the State of Illinois or LaSalle County. One Level 2 finding was identified and successfully re-demonstrated for the State of Illinois. There were no Level 2 :findings for the County of LaSalle. There was one previous Area Requiring Corrective Action from a previous exercise that was resolved for LaSalle County.

One Plan Issue from a previous exercise was resolved for both the State of Illinois and LaSalle County during the revision of plans and procedures.

Based on the results of the exercise, the planning and preparedness for the State of Illinois and affected local jurisdictions provide reasonable assurance that appropriate measures can be taken to protect public health and safety. Therefore, Title 44 CFR Part 350 approval of the offsite radiological emergency response plans and preparedness for the State of lliino~s remain in effect.

SUMMARY

On July 20, 2016, a Radiological Emergency Preparedness (REP) Partial-Participation Plume Exposure Pathway Exercise was evaluated by the U.S. Department of Homeland Security (DHS)/Federal Emergency Management Agency (FEMA) for the 10-mile Emergency Planning Zone (EPZ) around the LaSalle County Station (LCS). The purpose ofthiS exercise was to assess the capabilities of the State and local preparedness in responding to a radiological emergency. This exercise was held in accordance with DHS/FEMA's polis;ies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures.

The most recent exercise at this site was conducted on May 21, 2014. The qualifying emergency preparedness exercise was conducted on December '4', 1980.

The DHS/FEMA wishes to acknowledge the efforts of the many individuals who participated in this exercise. In the State of Illinois, the risk county of LaSalle and the support county of Grundy participated along with State Government.

Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities.

Cooperaticin and t'eamwork on the part of all the participants were evident during this exercise.

This Final After Action Report (AAR)/Improvenient Plan (IP) contains the evaluation of the biennial exercise and the evaluation of the following out-of-sequence interviews and activities:

Medical Services (MS-1) Hospital and Transportation evaluation at OSF Saint Elizabeth's Hospital Protective Actions for Schools (EV-2) for Marseilles Elementary School Ottawa Police Department TACP Ottawa Police Department DCO Briefing Sections 3 and 4 of this report provide detailed information regarding any Level 1 or Level 2 Findings, Plan Issues and Schedule of Corrective Actions.

After Action Report/Improvement Plan LaSalle County Station SECTION 1: INTRODUCTION 1.1 Exercise Basis On December 7, 1979, the President directed FEMA to assume the lead responsibility for all off site nuclear planning and response. The DHS/FEMA's activities are conducted pursuant to Title 44 of the Code of Federal Regulations (CFR) Parts 350 "Review and Approval of State and Local Radiological Emergency Plans and Preparedness", 351 "Radiological Emergency Planning and Preparedness" and 352 "Commercial Nuclear Power Plants: Emergency Preparedness Planning" (Commonly referred to as 44 CFR 350 through 352). These regulations are a key element in the Radiological Emergency Preparedness (REP) Program that was established following the Three Mile Island Nuclear Station accident in March 1979.

FEMA Regulation 44 CFR 350 establishes the policies and procedures for DHS/FEMA's initial and continued approval of State and local governments' radiological emergency planning and preparedness for commercial nuclear power plants. This approval is contingent, in part, on State and local governments' participation in joint exercises with licensee.

The DHS/FEMA's responsibilities in radiological emergency planning for fixed nuclear facilities include the following:

Taking the lead in offsite emergency planning and in the review and evaluation of Radiological Emergency Response Plans (RERP) and procedures developed by State and local governments; Determining whether such. plans and procedures can be implemented on the basis of observation and evaluation of exercises of the plans and procedures conducted by State and local governments; Responding to requests by the U.S. Nuclear Regulatory Commission (NRC) pursuant to the Memorandum of Understanding between the NRC and FEMA dated October 1, 2015, and; Coordinating the activities of Federal agencies with responsibilities in the radiological emergency planning process:

U.S. Department of Agriculture; U.S. Department of Commerce; U.S. Department of Energy; U.S. Department of Health and Human Services; U.S. Department of the Interior; U.S. Department of Transportation; U.S. Environmental Protection Agency; U.S. Food and Drug Administration; and U.S. Nuclear Regulatory Commission.

Representatives of these agencies serve on the DHS/FEMA Region V Regional Assistance Committee (RAC), which is chaired by DHS/FEMA.

After Action Report/Improvement Plan LaSalle County Station Formal submission of the RERP for the LCS to FEMA Region V by the State of Illinois and involved local jurisdictions occurred on March 31, 1981. Formal approval of these RERP was granted by F9MA on June 4, 1982, under 44 CPR 350.

A REP Partial-Participation Plume Exposure Pathway E.xe:rcise was conducted 'on July 20, 2016, and evaluated by the DHS/FEMA to assess the capabilities of State and local offsite emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency illvoiving the LCS, The purpose of this AAR/IP is to present the exercise results and fin.dings based on. the performanc.e of the Offsite Response Organizations (OROs) during a simulated radiological er:nergency.

The findings presented in this AAR/IP are based on the evaluations of the Federal evaluation team, with final determinations inade

  • by the DJiSIFEMA Region V RAC Chairperson, arid approved by the DHS/FEMA-Headquarters.

After Action Report/Improvement Plan LaSalle County Station 1.2 Emergency Planning Zone (EPZ) Description The LaSalle County Station is located on a 3,060-acre site in Brookfield Township, four miles west of the LaSalle and Grundy Counties line. It is located in the northeastern portion of Illinois, approximately seven miles south-southeast of the community ofMarseilles, four and one-half miles south of the Illinois River, which ruhs east-west through.LaSalle County and six miles southwest of the community of Seneca. The 10-mileplume extends outward in all directions from the LaSalle County Station and impacts portions of LaSalle and Grundy Counties. This EPZ includes the municipalities of Grand Ridge, Marseilles, Ransom and Seneca in LaSalle County, and Kinsman and Verona in Grimdy County.

The population distribution in this 101mifo EPZ is relatively low. Following is a list of the four largest communities in the EPZ and their respective 'year 2010 population figures: Kinsman (99), Marseilles (5,094), Seneca (2,371) and Verona (215). The year 2010 total population for these communities was 7,779 persons. The IO-mile EPZ is divided into 13 Sub-Areas.

Following is a list of the Sub-Areas with their associated year 2010 permanent resident population: Sub-Area I (1,060), Sub-Area 2 (77), Sub-Area 3 (784), Sub-Area 4 (3,145) Sub-Afea 5 {507), Sub-Area 6 (108), Sub-Area 7(695), Sub-Area 8 (515), Sub-Area) (308), Sub-Area 10 (8,192), Sub-Area 11(6,147), Sub-Area 13 (687) and Sub-Area 17 (288).

Primary. land use around the LaSalle County Station is agricultural, with crops of corn and soybean. The closest industries are focated in the communities of Marseilles and Seneca.

Recreational facilities. butside the municipalities include a sixty-acre park with camping facilities on the southwest shore of the 2058-acre cooling lake for the LCS. The cooling lake, which occupies over two thirds of the 3060-acre station site, has recreational boating and fishing.

Seneca area inarinas have* facilities for over 400 boats on the Illinois River, and are just over four miles north-northeast cifthe LCS. Five and one~half miles north-northwest of the LCS is the 510 acre Illini State Park with camping, boating; picnicking and fishing.

Major transportation facilities in the LCS EPZ arelimited to Interstate 80, nine miles north of the station and the Illinois Waterway, four and orie..:half miles north of the LCS. Marseilles Lock and Dam, at river mile 244.5, is seven miles northwest. Five rail lines also run through the LCS EPZ and their location in the EPZ is described in relationship to the LCS. The Burlington Northern and Santa Fe Railway Company has two rail lines, a spur line into the station from Ransom and the other line six miles from the station running northeast to southwest; the CSX Transportation, Inc. line is five miles north, the Illinois Rail Net line is nine miles west and the Norfolk Southern line is ten* miles to the south just outside of the EPZ.

SUMMARY

3.1 Exercise Purpose and Design The DHS/FEMA Region V Office evaluated the LCS REP Partial-Participation Plume Exposure Pathway Exercise conducted on July 20, 2016 to assess the capabilities of local emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving the LCS. The purpose of this report is to present the results and findings on the performance of the Offsite Response Organizations (OROs) during a simulated radiological emergency.

3.2 Exercise Objectives, Capabilities and Activities Exercise objectives and identified Capabilities/REP Criteria selected to be demonstrated are discussed in Appendix E "Exercise Plan." The Exercise Planning Team (EPT) selected objectives that focus on evaluating emergency response procedures, identifying areas for improvement, and fostering collaboration between the various OROs and stakeholders. This exercise focused on the following objectives:

ORO demonstration of effective Emergency Operations Management; ORO demonstration of effective Protective Action Decision Making; ORO demonstration of effective Protective Action Implementation; ORO demonstration of effective Field Measurement and Analysis; ORO demonstration of effective Emergency Notification and Public Information; 3.3 ORO demonstration of effective Support Operations and Facilities.Scenario Summary Appendix F "Scenario Details," contains a summary of the Exercise Scenario, a simulated sequence of events that was used as t~iy basis for invoking emergency response actions by OROs during the LCS REP Partial-Participatio1iPhi'm~'E:Xposure Pathway Exercise on July 20, 2016.

Results of a technical review of the scenario, submitted on May 19, 2016, indicate thatthe scenario was adequate to support demonstration of DHS/FEMA requirements, as well as criteria selected by the Offsite Response Organizations provided in the State's May 10, 2016, and revised May 23, 2016, extent-of-play submissions.

During the exercise, in addition to information and data provided through the LCS onsite scenario, controllers from the State of Illinois provided "inject messages" containing scenario events and/or relevant data to those persons or locations who would normally receive notification of such events.

These inject messages were the method used for invoking additional specific response actions by OROs.

Immediately after the update, the decontamination nurse picked up the contaminated scissors and proceeded to cut the clothes off of the upper body of the patient. There was no attempt to change, decontaminate or have the scissors surveyed prior to re-use. At I 04 I hours, the controller called a timeout and retrained the REA staff on contamination control processes and procedures. After Action Report/Improvement Plan LaSalle County Station APPENDIX E: EXTENT-OF-PLAY This appendix lists the exercise criteria which were scheduled for demonstration in the LaSalle County Station Radiological Emergency Preparedness Partial-Participation Plume Exposure Pathway Exercise on July 20, 2016, and the offsite Extent-of-Play agreement accepted by DHS/FEMA Region V on June 20, 2016. The exercise criteria, contained in the FEMA Radiological Emergency Preparedness (REP) Program Manual (January 2016), represent a functional translation of the planning standards and evaluation criteria ofNUREG-0654/FEMA-REP-1, Revl, "Criteria for the Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980.

Because the exercise criteria are intended for use at all nuclear power plant sites, and because of variations among offsite plans and procedures, an Extent-of-Play agreement is prepared by the State and accepted by DHS/FEMA to provide Evaluators with guidance on expected actual demonstration of the criteria.

The ability to direct and control emergency response activities will be demonstrated at the SEOC, REAC, the UAC (Simcell) and the nc. Coordination will be demonstrated between the SEOC, the REAC, the UAC (Simcell), the JIC, the EOF, and the county EOCs.

Criterion 1.d.l: At least two communicatiCmsystems are available, at least one operates properly, and communication links are established and maintained with appropriate locations.

Communications capabilities are managed in support of emergency operations.

IEMA will use a variety of communication systems [Nuclear Accident Reporting System

EVALUATION AREA 2 - PROTECTIVE ACTION DECISION MAKING.

Criterion 2.a.1: OROs *use a-decision-making process, considering relevant factors and appropriate coordination, to'ensurethat an exposure' control system, including the use of KI, is in place for emergency workers including provisions to authorize radiation exposure in excess of administrative limits or protective action guides..

The ability to make the decision to recorririiend the use of KI to emergency workers will not be demonstrated in REAC. This criterion was successfully demonstrated by the State in the last two years.

  • Criterion* 2.b.1: Appropriate protective *action recommendations are based on available information on plant conditions, field monitoring data and licensee and ORO dose projections, as well as knowledge of on-site and off-site environmental conditions.

The REAC will evaluate the licensee information and complete independent dose projections based on that information *and simulated field monitoring data.. REAC will make an evaluation of the data and make a protective action recommendation to the SEOC.

The coordination to determine protective action recommendations (P ARs) based upon available shelters, evacuation time estimates and other relevant factors will be demonstrated between the SEOC and REAC.

The coordination to determine protective action recommendations (PARs) based upon available shelters, evacuation time estimates and other relevant factors will be demonstrated between the SEOC and REAC. The State PAR will be transmitted to LaSalle County BOC and Grundy County BOC via NARS.

The capability to determine which centers should be activated for monitoring, decontamination, registration reception and congregate ~are will be demonstrated at UAC (Simcell), with input from the SEOC and REAC.

The Ambulance will use 2-way radios to communicate with OSF Saint Elizabeth. Other communication systems that may be used include commercial telephone or cell phones.

OSF Saint Elizabeth will adequately demonstrate the ability to support operations, with adequate resources. OSF Saint Elizabeth will also issue dosimetry and perform a dosimetry control briefing to staff prior to drill start.

EV ALAUTION AREA 6.d -TRANSPORTATION AND TREATMENT OF CONTAMINATED INJURED INVIDUALS Criterion 6.d. l: The facility/ORO has the appropriate space, adequate resources, and trained personnel to provide transport, monitoring, decontamination, and medical services to contaminated injured individuals.

Introduction:==

An offsite medical drill will be conducted to demonstrate the State of Illinois' concept of operations of handling contaminated injured individuals. The drill is structured to address MS., 1 Hospital and Transportation Criteria.

Extent of Play:

The LaSalle Nuclear Power Station has declared a general emergency. The emergency alert sirens have sounded, the public has been directed to evacuate affected areas ahd to report to reception centers set up in the local area. The scenario is based on a local resident who has reported to the reception center from a rural area and spent an extended time in the LaSalle EPZ.,

  • The drill will begin at the pickup of the patient by the ambulance. Any reception center activities
  • will be demonstrated at the reception center demonstration.

Hospital and Transportation:

The hospital will demonstrate procedures for limiting exposure to hospital staff, decontaminating a patient, and restricting access to the area where the patient is being treated and monitored.

Ottawa Fire will demonstrate the capability to transport a contaminated, injured individual to OSF Saint Elizabeth. The ambulance crew will pick up a.contaminated injured patient near the hospital at a simulated reception center. IEMA MRT staff will accompany the ambulance staff during the transportation portion of the drill.

Ottawa Fire will call in the information regarding the patient to hospital so they can prepare the receipt of a contaminated injured patient. OSF Saint Elizabeth will implement their plan for receipt, isolation and treatment of an injured contaminated patient. Medical personnel will utilize universal precautions and good housekeeping practices to minimize the spread of contamination, and will focus on treating the patienf s medical condition. Simple :

  • decontamination efforts will be demonstrated after the patient has been medically stabilized. The hospital will demonstrate procedures for limiting exposure to hospital staff; decontaminating a patient, and restricting access to the area where the patient is being treated and monitored.

Hospital personnel will demonstrate their knowledge of who to call beyond IEMA for assistance in Radiological Accidents, e.g., REAC/TS.

One IEMA MRT will be participating in this drill. One IEMA MRT is to facilitate monitoring the parent of the patient, ambulance staff and ambulance itself so it can be put back into service 60_:

The drill will conclude with hospital nuclear medicine staff supervising the removal of protective clothing and surveying of the emergency room and hospital personnel.

IEMA will also advise on the proper procedure for release or disposal of contaminated material.

Objectives:

1. Demonstrate the ability of EMS personnel to transport a contaminated injured patient.
2. Demonstrate the ability of hospital personnel to treat a contaminated injured patient.
3. Demonstrate the ability of personnel to exercise proper radiological controls and issuance of dosimetry.
4. Demonstrate the proper techniques of personnel decontamination.
5. Demonstrate proper communication between medical personnel and IEMA staff.
6. Demonstrate proper use of radiation detectors.

The scenario is based on a local resident who had stayed in the EPZ when the release occurred.

This drill will originate near the hospital at a simulated reception center.

1. An ambulance and EMS staff will be used to demonstrate loading, transporting and unloading the victim. EMS personnelwillpick up the patient at a staged location close to the hospital. The patient will be pre-staged for the ambulance arrival. IEMA staff will be present for the transportation portion of the drill.
2. The ambulance crew will communicate with the receiving hospital regarding the medical status of the patient and any additional precautions taken to prevent spread of contamination.
3. IEMA MRT and hospital nuclear medicine staff will be providing radiological exposure control and monitoring of EMS and hospital personnel.

4. Decontamination is determinant on ambulance protocols and injury that the patient presents.
5. Hospital nuclear medicine personnel will be responsible for any patient and staff radiological monitoring and contamination control activities until an IEMA MRT arrives on scene.
6. The IEMA MRT will supervise the ingress and egress ofradiological control areas.

Monitoring will be performed prior to personnel leaving the potentially contaminated patient treatment area. Protective clothing used by hospital personnel will be similar to that used for a chemical or biological agent in accordance with hospital protocol.

Multiple methods of decontamination, including dry, damp or wet, may be utilized for the removal of contamination.

7. The medical facility will demonstrate or describe their procedures for the medical treatment and necessary decontamination of a contaminated injured individual. Multiple methods of decontamination, including dry, damp or wet, may be utilized for the removal of contamination. !EMA/hospital nuclear medicine personnel will survey the hospital REA and medical personnel to maintain contamination control. These methods will include taking swipes of floors and surfaces so that the hospital and ambulance can be cleared for normal operations.
8. The hospital may need to contact REAC/TS to determine appropriate samples needed to assess internal contamination. Any samples collected will be sent to REAC/TS for analyzing, IEMA does not process biological samples.
9. Emergency medical personnel will be able to maintain their exposure below the limits specified in 10 CFR Part 20 because for the exercise, the dose rate from the patient is below 2 mr/hr.
10. After the Hospital is notified, hospital personnel will prepare the area to receive the patient in accordance with their procedures and provide security as necessary. IEMA as a general practice would, if necessary, post radiation signs in accordance with the requirements as set forth in 10 CFR Part 20. Hospital security will control the area in accordance with the same policies and procedures used to provide isolation in the treatment of a chemical or biological agent.
11. Regardless of specific written hospital procedures for addressing radiation contamination, the supervision and advice provided by IEMA personnel should be the governing guidance for determining whether the patient's contamination situation is appropriately addressed.
12. The contaminated injured patient will be accompanied by a parent that needs to be monitored. The pediatric decon will be an objective to be performed and evaluated by the hospital, per hospital request.

ALERT-0810 A magnitude 3.5 earthquake will strike with an epicenter 20km East of Morris, IL. The magnitude of ground motion is less than' the OBE (Operating Basis Earthquake) and SSE (Safe ShutdoWI1 Earthquake) levels * *

alarms will come in for the Reactor Building. When the isolation signal comes in it will attempt to close the 1E51-F063 (inboard RCIC Steam Isolation valve) and the 1E51-F008 (outboard RCIC Steam Isolation valve).

Isolation of the steam leak into the room will fail due to; 1E51-F063 breaker will blow a fuse when the isolation signal comes in which will fail the valve in the as-is position.

1E51-F008 valve will only achieve mid position based on a bent stem that was caused by the seismic activity.

The high temperature in the. RCIC Room win.cause the operators to scram Unit 1 in accordance with the LG As. The scram wiil result in ari ATWS at ;_ 3 0% reactor power due to hydraulic lock. The ATWS combined with the mechanical shock during.the earthquake will result in fuel damage that will cause the QryweURadiation level to being rising. Actions to reset ARI and _re-preform the scram from the Control Room will be successful.

The SRM D will f~il to insert.due to a bl~wn fuse which will be prioritized as a low level equipment issue by the ERO. Also, the SBGT system will start and run successfully. Actions to shutdown,the U2 SBGT system will be simulated complete.

The Station Emergency Director will declare a SITE AREA EMERGENCY (FS i) due to the Loss of RCS (RC.4.1*) and Loss of Containment (CT.6.1). This is from the unisolable RC!C line break and unisolable dife~t downstream pathway to the environment after a primary containment isolation signal.

An assembly will be initiated.by tl:ie ERO to. account for all personnel on site and evacuate non-essential personnel to an:9ffsite facility(either Mazon, Dresden or Braidwood)..

The Corporate Emergency Director will develop PARs to evacuate Sub-areas 1, 3 and notify the State of Illinois.
