ENCLOSURE 2 NOTICE OF DEVIATION Duke Power Company Docket No. 50-287 Oconee Nuclear Plant License No. DPR-55 During an NRC inspection conducted on October 3-7, 1994, a deviation of written commitments was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Action," 10 CFR Part 2, Appendix C, (1990), the deviation is listed below:
The Oconee Final Safety Analysis Report (FSAR), Section 7.2.3.3, Electrical Isolation, states, in part, that all signals leaving the reactor protective system (RPS) are isolated from the system either by the use of isolation amplifiers for analog signals, or by relay contacts in the case of digital signals. The effect of this isolation is to prevent faults occurring to signal lines outside of the RPS cabinets from being reflected into more than one protective channel.
Contrary to the above, on October 6, 1994, the NRC identified that Temporary Modification (TM) 1133 was implemented for Unit 3 on April 13, 1994, which connected a multi-channel chart recorder to RPS Channel C.
The TM did not provide electrical isolation from the RPS in accordance with FSAR Section 7.2.3.3 in that the recorder was plugged into a local 120 Volt AC non-isolated power source.
Please provide to the U.S. Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, D.C. 20555, with a copy to the Regional Administrator, Region II, and if applicable, a copy to the NRC Resident Inspector, in writing within 30 days of the date of this Notice, the reason for the deviation, the corrective steps which have been taken and the results achieved, the corrective steps which will be taken to avoid further deviations, and the date when your corrective actions will be completed.
Where good cause is shown, consideration will be given to extending the response time.
