ENCLOSURE 2 ENFORCEMENT CONFERENCE AGENDA U. S. NUCLEAR REGULATORY COMMISSION AND DUKE POWER COMPANY AUGUST 4, 1994 I.

INTRODUCTION AND OPENING REMARKS L. Reyes, Deputy Regional Administrator II. DISCUSSION OF THE ENFORCEMENT POLICY B. Uryc, Director, Enforcement and Investigation Coordination Staff III.

APPARENT VIOLATION AND NRC CONCERNS L. Reyes, Deputy Regional Administrator IV. LICENSEE DISCUSSION OF JUNE 14, 1994 LOCKOUT OF THE KEOWEE OVERHEAD PATH; THE SINGLE FAILURE VULNERABILITY; AND RELATED ISSUES:

J. Hampton, Vice President Oconee Nuclear Site Duke Power Company

1.

Circumstances surrounding the event (i.e., sequence of events, safety significance, etc...).

2.

Basis for initial modification which incorporated the automatic lockout of the overhead path on low Air Circuit Breaker (ACB) air pressure, as well as licensee awareness of the features prior to the event.

3.

Timeliness of operability determinations and corrective actions associated with the single failure vulnerability.

4.

Failure of previous programmatic efforts to identify the single failure vulnerability.

5.

Appropriateness of maintenance and testing on the ACB air system and check valves, including any changes since event.

6.

Lack of controlled drawings and valve identification tags for the ACB air supply system. (Endemic to other skid-mounted systems that contain safety-related equipment and components?)

QA NOTES To: Encoding Staff, eal Date:8/ 9 /94 From: GA ITEMS

1.

Backfit Y is not entered on any CF documentation. It is only picked up on applicable PORIPDR FCEN docs. For a further explanation, see your instructions in encoding manual under BFT.

2.

There should never be a transcript with a PDR 10CFR PT9.7 file & NRCZ in AA. If the transcript is an NRCC transcript, AA-NRCC & file-PT9.7. If the transcript has any other AA, you must assign another applicable file.

3.

Improvements noted: remembering to place apostrophe in the name O'Hanlon. Now, don't forget the apostrophe in O'Connor,P.W. name. See 9408030159.

4.

If a document discusses confirmatory action Itrs (CAL) in general (as opposed to a specific CAL

  1. ) you need to pick up CAL in OID.See your 0010 section of encoding manual & 9407290344 as an example.
5.

For an ANIRN that has three first name initials, drop the third first name initial & only pick up the first two witheir last name. Ex: Cortez,J.L.M. is entered as - Cortez,J.L.

6.

If you begin your TID w/ "Forwards" or a similar verb, do not end your TID w/ "W/encl" as it is repetitive & unnecessary.QA is noting "WIencl" being over-used.

7.

Encoders are forgetting line 10 dkt entries.

8.

When quoting a rept title wla hyphenated word in your TID,both parts of the hyphenated word should be initial-capped. Ex: Low-Enriched. See 9408020202.

9.

New Codes Administration is noticing that encoders are using affil that are crossed out on affil table instead of the proper affil handwritten next to the crossed-out affil. Use the handwritten affil. See attachment as an example.

10.

Misadministrations were once always CF proprietary. During a recent Blanton meeting, our procedures for processing misadmins has changed slightly - not all misadmins will be CF prop.

If they do not come in wla prop flag, encode them as just CF. As a general rule, if a person was misadministered, the doc will most likely be prop. Any other type of misadmin will most likely be just CF.
