ENCLOSURE NRC REVIEW OF MANAGEMENT ORGANIZATION AND TRAINING ISSUES The NRC staff conducted a review of selected documents concerning management issues at the Keowee Hydro Station (Keowee). This review was made as a result of NRC concerns related to the management of Keowee following the Loss of Offsite Power event which occurred at Oconee on October 19, 1992.

Specifically, the staff reviewed "Oconee Nuclear Station Operations Management Procedure 5-2 Duties and Responsibilities of Keowee Station Personnel,"

"Oconee Nuclear Station Operations Management Procedure 5-1 Keowee Station Log," "Keowee Maintenance Activities," and the Keowee "Job/Task Analysis and the Training Program Development Project Description" using Standard Review Plan 13.1.1, "Management and Technical Support Organization." The following comments concern three general areas: 1) communication practices between Oconee and Keowee, 2) chain of command and organizational structure at Keowee, and 3) training of Keowee personnel.

Communication Practices:

"Oconee Nuclear Station Operations Management Procedure 5-2 Duties and Responsibilities of Keowee Station Personnel" does not provide clear direction on communication practices and policies between Keowee and Oconee. Many of the personnel at Keowee (i.e., shift operators, technical personnel, and supervisors) can communicate rather freely with various Oconee staff. Further, these communication channels are dependent on the situation or plant status (i.e., normal, emergency, maintenance). The document does not clearly and consistently designate communication responsibility to specific staff, nor does it provide detailed descriptions of communication procedures and practices.

For example, Enclosure 4.1 of the above procedure states that "KHS shift operators communicate with ONS Unit 2 Supervisor/Control Room SRO," while.2 states that the "KHS supervisor is the primary contact for all off normal events and emergencies," and Enclosure 4.4 states that "During an emergency start, the KHS shift operator communicates with the ONS operations supervision and the 'On Call' Technical Personnel." It is apparent that the many channels of communication and responsibilities and duties of the Keowee staff, are often insufficiently descriptive, consistent, and explanatory. As a result, the probability of miscommunication and inaccurate transfer of information between Oconee and Keowee is increased.

Organizational Structure and Chain of Command:

The document "Oconee Nuclear Station Operations Management Procedure 5-2 Duties and Responsibilities of Keowee Station Personnel" does not provide clear organizational structure or lines of authority at Keowee.

For example, Enclosure 4.1, page 2, of the above procedure states that the "KHS Supervisor reports to the ONS Operations Superintendent both functionally and organizationally. The KHS Technical Personnel will report to the KHS Supervisor both functionally and organizationally. The KHS Shift Operator will report to the KHS supervisor both functionally and organizationally."

-2 However, Enclosure 4.1, page 2, states that the "KHS operator reports to the KHS station Supervisor or Technical Personnel in both on shift and off shift,"

while Enclosure 4.4 states that the "KHS operator reports to Supervisor and takes direction from the Technical Personnel," and Enclosure 4.4 states that "During an emergency, the KHS operator shall take direction from the 'On Call' personnel with approval from ONS Operations Supervision." These chains of command, particularly for the Keowee operator, are inconsistent across situations and poorly defined. As a result, communication and performance errors can occur.

Training of Keowee Personnel: The document "Job/Task Analysis and Training Program Development Project Description" provides plans for developing the training program at Keowee that are sound and well thought through. The project schedule is reasonable. However, the "Oconee Nuclear Station Operations Management Procedure 5-2, Duties and Responsibilities of Keowee Station Personnel" does not reflect these improvements in the training program.

For example, this procedure states that the "Keowee Technical Personnel are responsible for training the Keowee Shift Operators" (Enclosure 4.3), and that the "Keowee Station Supervisor is responsible for providing training to station personnel when needed" (Enclosure 4.2). These brief mentions of training responsibilities and practices may cause the training program to appear limited and inadequate. The above procedure should include references to the new training program development project description to reduce any misinterpretation of the quality of the training of Keowee staff.

Overall, the "Oconee Nuclear Station Operations Management Procedure 5-2 Duties and Responsibilities of Keowee Station Personnel" is inconsistent in some areas and often unclear and open to interpretation. The organizational structure and chain of command need to be more clearly defined and consistent across situations. Communication policies between Oconee and Keowee should be more structured and detailed to facilitate the accurate flow of information.

Finally, the new training program is adequate, but should be referenced in "Oconee Nuclear Station Operations Management Procedure 5-2 Duties and Responsibilities of Keowee Station Personnel."
