CONSTRUCT DEFICIENCY PART-21 REPORT - CLOSED SWITCH NOT ACTUATING WHEN VALVE IS FULLY CLOSED The following is a summary of a submitted facsimile:

"Items subject to this Fisher Information Notice (FIN) were provided to Catawba Nuclear Station (CNS) per CNS Order 00178457 (Fisher Order Number 033-D800306145). Specifically, Fisher supplied an 8 inch Design All Butterfly Valve with a Type 1052 Actuator and Top Worx Switches, serial number F000458892 ('Equipment'). The issue is with the closed switch not actuating when the valve is fully closed.

"The purpose of this FIN is to alert CNS that as of 2 September 2015, Fisher Controls International LLC (Fisher) became aware of a situation which may affect the performance of the Equipment, including its safety-related function.

Items subject to this Fisher Information Notice (FIN) were provided to Catawba Nuclear Station (CA(S) per CNS Order 00178457 (Fisher Order Number 033 -D800206145J.

Specifically, Fisher supplied an 30 Design All Butterfly Valve with a Type 1052 Actuator and Top Worr Switches, serial number F:000458802 ("Equipment"j. The Issue Is with the closed switch not actuating when the valve is fully closed.

Purpose; The purpose of this FIN is to alert CNS that as of 2 September 2015, Fisher Controls International LLC (Fisher) became aware of a situation which may affect the performance of the Equipment, including its safety-related function.

Fisher is informing CNS of this circumstance in accordance with Section 21.21 (b) of 10 CFR 21.

This FIN applies only to the subject Equipment supplied by Fisher Controls to ONS via the aforementioned order.

Discussion:

The switches are operated by Target Magnets on a bracket which is attached to a hub in the actuator housing. The hub rotates based on valve shaft rotation. The shaft and hub are indexed for proper disc positioning. For the subject valve, the proper alignment and indexing for disc/shaft pinning was incorrect. This misalignment resulted in a misalignment of the Target Magnets to the closed swItch, such that it would not actuate when the disc was fully closed.

The open switch was also mnisaligned, but not to an extent that prevents the switch from actuating.

This issue was discovered following receipt of the Equipment by CNS. Duke's diagnostic testing determined the closed limit switch was not actuating with the valve fully closed. Further examination was performed by Fisher Lifecycle Services Colombia, which confirmed this finding.

It is assumed that the ostensibly identical valve assembly provided on Order 033 -D800306145 (item CC2A) has successfully undergone functional testing and does not have the same defect.

Action Required:

Fisher will provide CNS with a properly pinned Disc/Shaft Assembly which will properly align Hub and Target Magnet Bracket allowing for proper switch actuation, in addition, a Corrective Action Request (CAR 1772) has been initiated by Fisher to prevent reoccurrence of this issue.

Discussion:

The switches are operated by Target Magnets on a bracket which is attached to a hub in the actuator housing. The hub rotates based on valve shaft rotation. The shaft and hub are indexed for proper disc positioning. For the subject valve, the proper alignment and indexing for disc/shaft pinning was incorrect. This misalignment resulted in a misalignment of the Target Magnets to the closed swItch, such that it would not actuate when the disc was fully closed.

The open switch was also mnisaligned, but not to an extent that prevents the switch from actuating.

This issue was discovered following receipt of the Equipment by CNS. Duke's diagnostic testing determined the closed limit switch was not actuating with the valve fully closed. Further examination was performed by Fisher Lifecycle Services Colombia, which confirmed this finding.
