SUBJECT:
TASK INTERFACE AGREEMENT (TIA) RELATING TO OCONEE PERFORMANCE DURING OUTAGES (TIA 92-02)
The enclosed memorandum from Warren Lyon of the Reactor Systems Branch of NRR reflects his observations regarding the operation of the Oconee nuclear units during plant shutdown and outage periods. These observations were made while participating in the AIT inspections conducted in September 1991. Many of the comments are subjective evaluations and as such are not fully included in the AIT report. Other comments are reflected by findings of the AIT.
NRC has taken a number of actions subsequent to the plant shutdown events in September 1991.
These actions included (1) meeting with the licensee to discuss deficiencies in the Oconee technical specifications, (2) a management meeting with senior licensee management to discuss operating performance at all three Duke nuclear stations, and (3) a shutdown risk pilot inspection at Oconee. In addition, a management meeting was held in Region II on September 25, 1991, and an Enforcement Conference was conducted on December 18, 1991 to discuss the apparent violations identified by the AIT.
As discussed previously, we are requesting that Region II evaluate each of the concerns in the enclosure in light of previous inspection and other activities as discussed above at Oconee relating to shutdown risk and determine whether additional inspection activity or other action is needed to address these concerns. The Division of Reactor Inspection and Safeguards has agreed to work closely with Region II to identify areas appropriate for evaluation as part of the ongoing shutdown risk pilot inspection. Also enclosed is a matrix identifying those issues being addressed by this inspection.
It is requested that you address each of the items identified in the attached memorandum, consider additional inspection activity that may be required, and include any previous activities that would resolve the issue. Many of the comments are subjective evaluations and, as such, are not fully included in the AIT report. Other comments are reflected by findings of the PIT.
NRR has taken a number of actions subsequent to the plantPshutdown events in September 1991. These actions include meetings with the licensee to discuss deficiencies in the Oconee technical specifications, a management meeting with senior licensee management to discuss operating performance at all three Duke nuclear stations, and scheduling a shutdown risk pilot inspection at Oconee.
In addition, we are aware of the management meeting held in Region I on September 25, 1991, and the Enforcement Conference conducted on December 18, 1991, to discuss the apparent violations identified by the AT.
In response to th concerns identified by Warren Lyon in his memorandum, we have undertaken the following actions:
- 1. Performed a comparison between the AIT report and the observations in Mr. Lyon's memorandum to determine pre-identified issues.
- 2. Provided Mr. Lyon's observations as background material for the pilot shutdown risk inspection.
- 3. Provide Mr. Lyon's memorandum to Region 11 for your evaluation and action as appropriate.
Enclosed are the results of our comparison of pre-identified issues (Action 1 above). It is requested that the enclosures be reviewed by Region d to determine if any additional action(s) on the part of the NRC is warranted.
