SUBJECT:
MEMORANDUM FROM WARREN LYON CONCERNING OCONEE SHUTDOWN OPERATIONS By memorandum dated November 21, 1991, Warren Lyon provided his observations and perspective on the operation of the Oconee Nuclear Station, particularly during outages. I feel that his input is particularly valuable at this time, as my staff and I share many of the concerns reflected in his memorandum.
As a result of our concerns, we have initiated a number of actions with the licensee. On September 25, 1991 a management meeting was held in Region II at which Duke Power Company (DPC) discussed the investigation and corrective actions implemented as a result of the September 1991 shutdown events. On October 9, 1991, a meeting was held at NRR to discuss deficiencies identified in the Oconee Technical Specifications in the area of shutdown requirements.
Corrective actions for these deficiencies were committed to by the licensee subsequent to this meeting. A meeting with senior licensee management was held on November 12, 1991, during which DPC organizational structure and operating performance were primary areas of discussion. An Enforcement Conference is scheduled for December 18, 1991, to discuss violations identified as a result of the shutdown events at Oconee Unit 1. Many of the issues raised by Mr. Lyon have been addressed by the licensee in these meetings.
In addition, as a result of the numerous concerns identified during the Augmented Inspection Team (AIT) review at Oconee, NRR has scheduled a pilot shutdown risk team inspection at Oconee. This will be a major team inspection, beginning the week of December 2, 1991, covering all aspects of outage activities.
To ensure that all of the concerns covered by Mr. Lyon are reviewed, we are taking the following additional actions:
- 1. Conduct a detailed comparison between the AIT report and the observations in Mr. Lyons memorandum to determine which areas have common findings.
- 2. Provide Mr. Lyon's observations, with the results of our comparison, to Region II for their evaluation and follow-up action as appropriate.
