Special Report: Containment HiQh RanQe Radiation Monitor Instrumentation Channel #12 Inoperable In accordance with Action 3 of Nine Mile Point Unit 1 (NMP1) Technical Specification (TS)

Table 3.6.11-2, "Accident Monitoring Instrumentation Action Statements," Nine Mile Point Nuclear Station, LLC (NMPNS) is submitting the following Special Report concerning inoperability of Containment High Range Radiation Monitor Instrumentation Channel #12.

Description of Event On June 6, 2015 at 0000, the Channel #12 Containment High Range Radiation Monitor was declared inoperable due to an incomplete monthly instrument channel test required by Surveillance Requirement (SR) 4.6.11.

The redundant channel of Containment High Range Radiation Monitor Instrumentation, Channel #11, was operable during the period Channel #12 was inoperable.

Cause of Inoperabilit, The cause of the inoperability was the inability to complete the monthly instrument channel test required by Surveillance Requirement (SR) 4.6.11. The section of the test that could not be performed is to check the "Trip Alarm 1" setpoint. The "Trip Alarm 1" outputs to a light located on the instrument. The remaining portions of the test were performed satisfactorily.

The "Trip Alarm 1" setpoint was found to be out of calibration low, which caused the light to come in unexpectedly.

Action Taken Instrument maintenance technicians adjusted the potentiometer associated with "Trip Alarm 1" and re-performed the monthly instrument channel test. The test was completed satisfactorily, and the Containment High Range Radiation Monitor Instrumentation Channel #12 was restored to operable on June 11,2015 at 1237.
