June 16, 2015 EN 15-020 OFFICE OF ENFORCEMENT NOTIFICATION OF SIGNIFICANT ENFORCEMENT ACTION Licensee: Susquehanna Nuclear, LLC EA-15-022 Susquehanna Steam Electric Station, Units 1 and 2 Docket No.: 05-387,05-388

ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violation (NOV) for a violation associated with a White Significance Determination Process finding will be issued on or about June 22, 2015, to Susquehanna Nuclear, LLC (licensee) as a result of an inspection. The White finding, an issue of low to moderate safety significance, will result in additional NRC inspections.

The White finding involves the licensees failure to implement the 15-minute assessment, classification, and declaration period for a potential loss of Reactor Coolant System (RCS) barrier emergency action level (EAL) at Susquehanna Steam Electric Station (Susquehanna),

Units 1 and 2. Specifically, when evaluating the potential loss of the RCS barrier, Susquehanna personnel were trained to not consider the EAL thresholds as being met unless operator mitigating actions, as identified in an emergency operating procedure for secondary containment control, were unsuccessful in isolating the leak. Susquehannas interpretation of the 15-minute assessment, classification, and declaration clock to start when operator actions were, or were expected to be, unsuccessful in isolating an RCS leak potentially degraded their ability to make timely Site Area Emergency declarations.

A NOV is included based on the violation of 10 CFR 50.47(b)(4), 10 CFR 50.54(q)(2), and 10 CFR 50, Appendix E, Section IV.C.2 requirements. The Notice is considered escalated enforcement action because it is associated with a White finding.

The NRC issued a preliminary White finding on April 16, 2015. In a letter dated May 15, 2015, Susquehanna provided a written response to NRCs preliminary determination and provided additional information. After careful consideration of the information developed during the inspection and the additional information provided by the licensee, the NRC has concluded that the finding is appropriately characterized as White. It should be noted that the licensee has not been specifically informed of the enforcement actions. The schedule of issuance and notification is:
