Topic 1 Topic 1 Description Program and System Management For the reporting period of 2014, fifteen (15) individuals were denied unescorted access authorization after testing positive for drugs or alcohol. By test type, one (1) follow-up test was positive, two (2) for cause - observed behavior tests were positive, nine (9) pre-access tests were positive, and three (3) random tests were positive.

Follow-Up Positive Test One (1) contractor employee tested positive for marijuana. The individual was denied access.

For Cause - Observed Behavior Positive Test One (1) licensee employee tested positive for alcohol. The individual was denied access. One (1) contractor employee tested positive for marijuana. The individual was denied access.

Topic 2 Topic 2 Description Certified Laboratories For the 2014 reporting period, all blind specimens submitted to St Lucie nuclear plant's HHS-certified laboratory yielded expected results.

One (1) specimen was lost in transit by the DHHS certified laboratory. The individual was retested with negative results.

Thirteen of the waivers were related to an emergent down power to support a recovery of foreign material in the Unit 2 B Operating or on- Performing Performing steam generator bowl. The waivered individuals performed activities to support SIG nozzle dam removal, installation of SG manways and Performing site directing of health physics maintenance or Performing tube plugging evaluations to enable exiting RCS midloop conditions. One additional waiver was necessary for a valve repair by a duties of a fire Furmanite Technician with unique qualifications. Analysis of Fatigue Assessment Data:

Eight online fatigue assessments were conducted in 2014:1) Five workers were assessed "For-Cause," two (2) Security Officers, two (2)

Contractor workers, and one(1) Maintenance worker. Conclusions:

There were a total of fourteen waivers issued in 2014. The waivers were required to address circumstances that could not have been reasonably controlled and to ensure the industrial or radiological safety of the public and plant personnel.
