21.21 (d)(3)(i) DEFECTS AND NONCOMPLIANCE PART 21 REPORT - INCORRECT VALVE SOLENOIDS PROVIDED The following is a synopsis of information received via facsimile:

Brunswick Nuclear Plant ordered 10 (ten) 120VAC/6OHz valve solenoids on two purchase orders from Fisher Controls International LLC. In February, 2014, Fisher shipped six 125VDC solenoids to Brunswick. All six of those solenoids were identified as non-conforming prior to installation in the plant. Purpose:

The purpose of this Fisher Information Notice (FIN) is to alert Progress Energy-Brunswick that as of 26 June, 2014, Fisher Controls International LLC (Fisher) became aware of the possibility of a situation which may affect the performance of the applicable equipment.

We are informing you of this circumstance in accordance with Section 21.21 (b) of 10CFR21.

Applicability:

This notice applies only to the subject equipment supplied by Fisher, identified above, LFISRIERJ Page 1of 2

Discussion Fisher has determined that the subject items did not meet the customer requested construction per the Customer Purchase Orders.

Specifically the discrepancies are:

  • Both Customer Purchase Orders specified 120VAC/6OHz solenoids. Fisher provided solenoids that were 125VDC.

A total of 10 items were ordered between the two Purchase Orders. A quantity of 6 were shipped in February 2014 but all were identified as non-confon-ning prior to installation in the plant. The remaining 4 are on a shipping hold pending solenoid replacement.

Action Required Fisher will support Progress Energy -Brunswick in performing the replacement of the solenoid valves at no cost to Progress Energy-Inaddition, a Corrective Action Request (CAR) is being initiated by Fisher to prevent this issue from reoccurring.

10CFR21 Implications:

Fisher requests that the recipient of this notice review it and take appropriate action in accordance with 10CFR21.
