Byron License Renewal (LR) Onsite Document Request (Dated 7-8-14)

To facilitate the scheduled NRC inspection in accordance with IP 71002 License Renewal Inspection, please provide the following documents to the lead inspector -Mel Holmberg (630) 829-9748 upon his arrival at the site on August 4, 2014.

A. For each Aging Management Program (AMP) and in-scope structure, system or component (SSC) provide: (as applicable- e.g. new programs may not have audits/self-assessments).

1) LR Scoping/Boundary Drawing(s) - Hard copy E-size.
2) LR Basis Documents for the AMP.
3) Program Health Reports for the AMP - Last two.
4) Self-Assessment Reports of the AMP - Last two.
5) Quality Assurance Audits of the AMP - Last two.
6) LERs associated with SSC managed by the AMP - Past 5 years.
7) List of site and fleet procedures and PM tasks with titles/descriptions/revisions that implement the AMP.
8) List with short description of the surveillance test records, preventative maintenance, corrective maintenance records, equipment history files, inservice test or inspection results that were reviewed by the applicant to determine or identify aging effects managed by the AMP.
9) Identify the industry operating experience data bases (e.g. INPO, NRC generic communications, vendor notifications etc.) searched and how these searches were conducted to identify aging effects managed by the AMP.
10) Upgrades to SSC that are planned, but not yet implemented.
11) SSC that have been monitored iaw 10 CFR 50.65(a)(1) of 10 CFR 50.65, Requirements for Monitoring the Effectiveness of Maintenance at Nuclear Power Plants within the past 3 years.
12) Provide the most current system health report for each system identified in Item 11.
13) Identify the list (if any) of in-scope SSCs identified after initial submittal of the Byron License Renewal Application.

B. License Renewal Technical Basis Document for the Regulated Events identified in 10 CFR 54.4(a)(3). If not already provided within these documents, identify each system or structure relied on to demonstrate compliance with NRC regulated events as discussed in 10 CFR 54.4(a)(3) and identify the function(s) provided by these systems or structures associated with the applicable regulated event(s).
