CONFIRMATION OF ACTION - DOCKET NO. 50-261 This letter is to confirm our understanding of commitments made during a telephone conversation between D. Verrelli, NRC Region II and C. Dietz, CP&L on September 1, 1992.
On July 9, 1992, H. B. Robinson was shutdown after testing revealed that the B safety injection (SI) pump's recirculation flow rate was significantly degraded. This degradation was attributed to the blockage of the pump's recirculation line orifice by pieces of white plastic material.
Following a reactor trip on August 22, 1992, testing of the SI pumps revealed that the recirculation flow rate was significantly degraded on the B SI pump and reduced by 10 percent on the A SI pump. As recent inspections have found pieces of white plastic in the B SI pump recirculation line orifice and the refueling water storage tank, it is apparent that the problem identified in July has not been fully resolved.
With regard to this situation, it is our understanding that you have committed to the following:
(1) Submit for NRC review a detailed recovery plan which identifies the root cause of the problem, as well as those actions taken to restore affected systems and preclude recurrence (2) Meet with the NRC in our Atlanta Office to discuss results of the recovery plan activities and the root cause of the problem prior to the unit entering the Hot Shutdown condition.
If your understanding of this matter differs from that stated above, contact this office promptly.
