SUBJECT: NOTICE OF VIOLATION
(NRC EXAMINATION REPORT NO. 50-261/96-300)
Gentlemen:
This refers to the Nuclear Regulatory Commission (NRC) administered
examinations led by Ronald F. Aiello during the period February 9-15, 1996.
The examinations were administered to employees of your organization who had
applied for licenses to operate the H. B. Robinson Steam Electric Plant. At
the conclusion of the examination, the examinations and preliminary findings
were 'discussed with those members of your staff identified in the NRC
Examination Report, Enclosure 2.
A Simulator Facility Report is included in this report as Enclosure 3. A copy
of the written examination questions and answer key, is included in this
report as Enclosure 4. Facility comments to the written examination is
included as Enclosure 5. The NRC's resolution to the facility comments is
included as Enclosure 6.
In October 1992, eight candidates from your facility participated in the
Generic Fundamentals Examination. Of the eight examined, eight passed for a
100 percent pass rate. In October,' 1993, three candidates participated. Of
the three examined, three passed for a 100 percent pass rate. In 1994, no
candidates participated. In April 1995, six candidates participated. Of the
six examined, six passed for a 100 percent pass rate.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violation is of concern because inadequate corrective
action with respect to EOPs and AOPs can affect the safe operation of the
plant and potentially endanger the health and safety of the public.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. In your
response, you should document the specific actions taken and any additional
actions you plan to take to prevent recurrence. After reviewing your response
to this Notice, including your proposed corrective actions and the results of
future inspections, the NRC will determine whether further NRC enforcement
action is necessary to ensure compliance with NRC regulatory requirements.
