SUBJECT:
(NRC INSPECTION REPORT NO. 50-261/93-34)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
the Augmented Inspection Team during the period November 20 through
December 6, 1993. The inspection objectives included review of the scope and
causes of the events which were revealed during the November 1993 post
.refueling
startup of H. B. Robinson Steam Electric Plant, Unit 2. The report
documenting this inspection was sent to you by letter dated January 5, 1994.
As a result of this inspection, failures to comply with NRC regulatory
requirements were identified, and accordingly, NRC concerns relative to the
inspection findings were discussed during an enforcement conference held on
March 14, 1994, in the NRC Region II office, Atlanta, Georgia. The
Enforcement Conference Summary was sent to you on March 17, 1994.
The violations described in the enclosed Notice of Violation involved the
failure to establish and implement procedures for refueling operations and
plant startup and the failure to assure that purchased fuel conformed to
It appears that a lack of effective management control and oversight of your
refueling and startup activities was the major factor contributing to the
violations, as evidenced by the examples of inadequate or unimplemented
procedures. Also, quality assurance activities failed to identify the
deficiencies in nuclear fuel procurement. Additionally, your lack of specific
procedural requirements led to the failure of the operating crew to monitor
diverse instrumentation readings during the unit startup and subsequently led
them to exceed the Technical Specification limits for startup rate.
Therefore, please include in your response your corrective actions to assure
adequate monitoring of instruments during unit startup evolutions.

You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response. In your
response, you should document the specific actions taken and any additional
actions you plan to take to prevent recurrence. After reviewing your response

to this Notice, including your proposed corrective actions and the results of
future inspections, the NRC will determine whether further NRC enforcement
action is necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of
this letter and its enclosure will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, Pub. L. No.96-511.
