CORPORATE/H. B. ROBINSON UNIT NO. 2 MOTOR OPERATED VALVE INSPECTION
(NRC INSPECTION REPORT 50-261/91-201)
We are forwarding the report of the motor-operated valve (MOV) team inspection
conducted by the U. S. Nuclear Regulatory Commission's (NRC's) Office of
Nuclear Reactor Regulation (NRR) from June 10 through 14, 1991, at the Carolina
Power & Light (CP&L) nuclear engineering offices in Raleigh, North Carolina.
This inspection reviewed the programs you developed in response to NRC Generic
)
Letter (GL) 89-10, "Safety-Related Motor-Operated Valve Testing and Surveil
lance."
Implementation of your corporate program was reviewed for the H. B.
Robinson Steam Electric Plant Unit No. 2 (H. B. Robinson 2).
The areas
examined during the inspection are discussed in the enclosed inspection report.
The results of this inspection were discussed with your staff during the exit
meeting on June 14, 1991.
The team found that portions of an acceptable MOV
program had been developed by CP&L, but identified many weaknesses, especially
with the H. B. Robinson plant implementing procedures. Overall the team
identified nine significant weaknesses. Of specific concern was CP&L's failure
to properly document and evaluate discrepant conditions found on two
safety-related MOVs. A similar concern was identified during a previous MOV
inspection at H. B. Robinson 2 (NRC Inspection Report 50-261/89-200), and the
continued failure to properly document and evaluate conditions that question
component or system operability is a problem that warrants increased management
attention. Other weaknesses identified by the team included inadequate proce
dures for controlling the performance and evaluation of diagnostic testing and
a setpoint document that did not contain minimum thrust requirements or limit
switch or torque switch settings for a large number of valves. These combined
weaknesses indicate a lack of overall management attention and priority to
MOV issues at the Robinson site. The team also noted that the CP&L stance on
valve mispositioning is not in conformance with the recommendations of GL 89-10.
Strengths related to the MOV program were identified in the areas of training,

involvement, and the electrical reviews related to thermal overload sizing,
motor performance in high ambient temperature conditions, and degraded voltage.
Please inform us in writing, within 60 days, when the actions to address the
open items contained in Appendix A to the enclosed inspection report will be
available for reinspection. In accordance with 10 CFR 2.790(a), a copy of this
letter and the enclosure will be placed in the NRC Public Document Room. Any
enforcement actions resulting from this inspection will be issued by Region II
under separate correspondence.
