Dear Sir:

This letter describes a reportable occurrence, as defined in Section 6.9.2.a(9) of Appendix A to the Provisional Operating Licence DPR-13.

After the installation of the new Containment Isolation Panels (13A and 13B) a problem was identified which potentially could cause the grounding of circuits when replacing burned out indicating lights. Since it is station practice to replace indicating lights with the power supply energized, the potential exists for operating certain containment isolation valves during the lamp replacement.

The switch type in question is Micro Switch Series 2 which is manufactured by Honeywell. Discussions with the vendor have been initiated to correct this deficiency. Station personnel have been instructed by station memorandum regarding those required procedural steps that must be performed prior to lamp replacement.

A complete report on this matter together with a License Event Report will be forwarded within two weeks.
