VIRGIL C. SUMMER NUCLEAR STATION (VCSNS) UNIT 1 DOCKET NO. 50-395 OPERATING LICENSE NO. NPF-12 SPECIAL REPORT (SPR) 2014-001 South Carolina Electric & Gas Company (SCE&G) is submitting this Special Report pursuant to the requirements of Technical Specification 6.9.2 as required by Action 30 of Technical Specification 3.3.3.1.b, Table 3.3-6, Item 2.d.ii. This report addresses the number of operable channels for radiation monitoring instrumentation less than required by the Minimum Channel operable requirement for more than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.
On March 21, 2014, at 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br />, the Main Steam Line B Radiation Monitor (RM-G19B) was entered into the VCSNS corrective action program and declared inoperable due to excessive failure alarms with no error code on the Universal Digital Ratemeter. Upon declaring RM-G19B inoperable, VCSNS implemented a preplanned alternate method of monitoring in accordance with the requirements of Action 30 of the Technical Specification. After extensive trouble-shooting, the failure of RM-G19B was determined to be due to a failed detector/preamp assembly. A refurbished detector/preamp was installed on March 25, 2014. RM-G19B was monitored overnight for failure alarms and was declared operable on March 26, 2014, at 1115 hours0.0129 days <br />0.31 hours <br />0.00184 weeks <br />4.242575e-4 months <br />.
