Dear Sir:

This letter describes a reportable occurrence as defined In Section 6,9.2.a(9) of Appendix A to the Provisional Operating License DPR-13.

During surveillance testing two snubbers located inside containment failed functional testing. The first, a main feedwater piping snubber, failed due to eKcessive hydraulic fluid leakage around the piston rod seal.

The second, a main steam piping snubber, had failed threads and a locking nut on the piston rod assembly. In accordance with Technical Specification 4.14, additional functional testing has been initiated.

A complete report on this matter, together with a Licensee Event Report, will be forwarded to your office within two weeks.
