Docket Nos. 50-206, 50-361 and 50-362 Follow-up Report, Inoperable Fire Doors - May 1990 San Onofre Nuclear Generating Station, Units 1, 2 and 3 This letter provides a follow-up report for Technical Specification (TS) fire-rated assemblies (i.e., fire doors) found inoperable during routine surveillances in May 1990. This monthly report and previous monthly reports provide updated information associated with the initial 1983 report for Units 2 and 3.

The enclosed report identifies: 3 closure mechanism failures and 3 latching mechanism failures for Unit 1; 17 latching mechanism failures and 7 closure mechanism failures for Units 2 and 3. As each of these deficiencies was identified, compensatory fire watches were established pursuant to TS 3.14.7, Action "A" for Unit 1, and TS 3.7.9, Action "a" for Units 2 and 3.

There were no Unit 1 or Unit 3 fire doors found to be blocked open in May. On May 7, 1990, Unit 2 fuel handling building (FHB) fire door FH2-304 was found blocked open during a routine fire door surveillance. The door was blocked open during removal of sealing equipment from the FHB as part of the re-racking project work. The responsible party was identified and appropriate disciplinary action was taken. All personnel involved in re-racking project work were re-instructed in the requirements prior to blocking open of fire doors.
