Docket Nos. 50-206, 50-361 and 50-362 Follow-up Report, Inoperable Fire Doors - February 1990 San Onofre Nuclear Generating Station, Units 1, 2 and 3 The purpose of this letter is to provide a follow-up report for Technical Specification (TS) fire-rated assemblies (i.e., fire doors) found inoperable during routine surveillances in February 1990. This monthly report and previous monthly reports provide updated information associated with the initial 1983 report for Units 2 and 3.

The enclosed report identifies: a) 2 latching mechanism failures and 2 closure mechanism failures for Unit 1 and; b) 34 latching mechanism failures and 13 closure mechanism failures for Units 2 and 3. As each of these deficiencies was identified, compensatory fire watches were established pursuant to TS 3.14.7, Action "A" for Unit 1 or TS 3.7.9, Action "a" for Units 2 and 3.

On February 20, 1990, four Unit 2/3 (common) battery room fire doors (AC-321, AC-322, AC-323, and AC 324) were found blocked open during a routine fire door surveillance walkdown. The doors were blocked open as required by procedures in order to maintain battery room temperatures during maintenance activities in the battery rooms (normal ventilation was not working properly). A compensatory fire watch was not established prior to blocking open the doors.

It should be noted, however, that the doors remained in this condition without the compensatory fire watch for only approximately 10 minutes. The individual responsible for blocking open the doors was identified and appropriate disciplinary action was taken.
