Docket Nos. 50-206, 50-361 and 50-362 Followup Report, Inoperable Fire Doors - January 1990 San Onofre Nuclear Generating Station, Units 1, 2 and 3 The purpose of this letter is to provide a follow-up report for Technical Specification (TS) fire-rated assemblies (specifically fire doors) found inoperable during routine surveillances in January 1990. This monthly report and previous monthly reports provide updated information associated with the initial 1983 report for Units 2 and 3.
The enclosed report identifies: a) 3 latching mechanism failures and no closure mechanism failures for Unit 1 and; b) 31 latching mechanism failures and 8 closure mechanism failures for Units 2 and 3.
As each of these deficiencies was identified, compensatory fire watches were established pursuant to TS 3.14.7, Action "A" for Unit 1 and TS 3.7.9, Action "a" for Units 2 and 3.
For this report month, one fire door (AC-326) which is common to Units 2 and 3 was found to be blocked open without proper authorization.
The responsible party was identified and appropriate disciplinary action was taken.
