MEETING

SUMMARY

OF SEPTEMBER 15, 1981 On September 15, 1981 members of the NRC staff met with representatives of Southern California Edison Co. and their consultants to discuss the September 3, 1981 event at San Onofre Unit 1. During this event, two safety injection discharge valves did not operate as designed. A list of attendees is enclosed along with copies of the viewgraphs presented by SCE.

The valves that failed to operate properly are hydraulically operated 14" double.disc wedge type gate valves. The licensee concluded that the pressure differential across the gate was sufficient to prevent the valves from opening. Subsequent lapping of the galled valve faces resulted in them operating when first retested but then failing to meet the design operating time on subsequent tests.

The licensee proposed corrective actions that they feel will resolve the problem. These actions principally involved tripping the feedwater (ECCS) pump on a safety injection signal to reduce the pressure on the upstream side of the disc of the affected valves.

The pump would be restarted within five seconds after the pressure decayed sufficiently to assure valve operation. Other actions include revised valve sequencing, steps to equilize pressure across discs and between lines and pressure limits on piping downstream of the valves.

The licensee indicated that they expected to be able to return the unit to service on September 24, 1981, since all modifications would be completed and licensee reviews would be completed by then.

The staff indicated to the licensee that they had some concerns associated with the proposal.

Included in these concerns is the potential added risk of stopping and restarting the feedwater (ECCS) pumps, the affect on the ECCS water and the startup and surveillance testing of the modified system.

The staff expects that additional discussions related to these matters will occur.
