Docket No. 50-206 Special Report San Onofre Nuclear Generating Station, Unit 1 Pursuant to Section 6.9.3 of Appendix A, Technical Specifications to Provisional Operating License DPR-13 for San Onofre Unit 1, this submittal provides the required 14-Day Special Report for inoperable radiation monitoring instrumentation.

On April 24, 1985, at 1725, with Unit 1 in Mode 1 at 93% power, Stack Wide Range Gaseous Monitor R-1254 was declared inoperable when its mid-range channel was observed responding erratically. The low-range and high-range channels were responding properly. The preplanned alternate method of monitoring was initiated per Limiting Condition for Operation (LCO) 3.5.10, Table 3.5.10-1, Action Statement 27.

Also, in accordance with LCO 3.5.9, Table 3.5.9.1, Action Statement 24, stack flowrate estimates every four hours during planned releases were initiated since the stack flowrate, a function of R-1254, was also declared inoperable.

Subsequent investigation has determined the erratic Mid-Range channel behavior was due to a faulty detector/preamp assembly connector. The detector/preamp assembly has been replaced. Calibration of Monitor R-1254 is still in progress. It is anticipated that Monitor R-1254 will be returned to operable status by May 12, 1985.
