APPENDIX A NOTICE OF VIOLATION Southern California Edison Company Docket No. 50-206 San Onofre Unit 1 License No. DPR-13 As a result of the inspection conducted during the period of February 8 through April 4, 1987, a violation of NRC requirements was identified. In accordance with the General Statement of Policy and Procedure for NRC Enforcement Actions, 10 CFR Part 2 Appendix C, the violation is set forth below:

Unit 1 Technical Specification 6.5.2, Technical Review and Control, states:

"Proposed modifications to unit nuclear safety related structures, systems and components shall be designed by a qualified individual/organization. Each such modification shall be reviewed by an individual/group other than the individual/group which designed the modification, but who may be from the same organization as the individual/group who designed the modification. Proposed modifications to nuclear safety related structures, systems and components shall be approved prior to implementation by the Station Manager; or by the Manager, Technical as previously designated by the Vice President and Site Manager, Nuclear Generation Site. Documentation of these activities shall be provided to the Vice President and Site Manager, Nuclear Generation Site, and to the NSG."

Contrary to the above, on February 2, 1987, the backup nitrogen system for the pressurizer relief valve block valves (CV530 and 531) had wood spacers under the bottles.

Design documents did not provide for these wood spaces.

This modification had not been reviewed by cognizant station personnel nor approved by cognizant station management.

This is a Severity Level IV Violation (Supplement I) applicable to Unit 1.

As noted in inspection report 50-206/87-03, Southern California Edison Company has fully implemented corrective actions to preclude recurrence of the above violation. Accordingly, a specific response to this Notice is not required.
