APPENDIX A Southern California Edison Company P. 0. Box 800 2244 Walnut Grove Avenue Rosemead, California 91770 Docket No. 50-206 Notice of Violation Based on the results of the NRC inspection conducted on March 13-15, 1979, it appears that one of the activities at your San Onofre Unit 1 facility was not conducted in full compliance with the conditions of NRC License No. DPR-13 as indicated below. This item is a deficiency.
10 CFR 50.54(i-1) states, in part, "Within three (3) months after issuance of an operating.license, the licensee shall have in effec.t an operator requalification program which shall, as a minimum, meet the requirements of Appendix A of Part 55 of this chapter."
Appendix A of 10 CFR 55 states, in part, "The requalification program shall include on-the-job training so that each licensed operator and service operator is cognizant of facility design changes, procedure changes, and facility license changes."
Paragraph 3.1.4 of your approved requalification program states that licensed operators will review facility design changes and will document these reviews on Form PSSD-121.
Contrary to the above, on March 15, 1979, documents were not available to show that your licensed operators had reviewed Facility Design Change Nos. 78-16, 78-17 and 78-18. These facility changes were completed during the 1978 refueling outage.
