APPENDIX A Notice of Violation Southern California Edison Company Docket No. 50-206 Based upon the results of the NRC inspection conducted on April 15-18, 1980 certain activities conducted under your license appear to be in noncompliance with NRC requirements as identified below.
I. Technical Specification Section 6.11 requires that written procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure.
Procedure S-VII 1.4. Rev. 5, April 1979, "Entering and Leaving Steam Generators", (a)Section III B2 requires that airborne particulate and iodine levels be determined for each steam generator involved prior to entry, and (b)Section III E2 requires that the worker who enters a steam generator stand still after exiting, and that the assigned Radiation Protection technician remove items of special protective clothing and equipment.
Contrary to the above, on April 12, 1980, work involving entry into steam generators "A", "B" and "C" was performed and airborne activity surveys were not performed for each generator involved and the special protective clothing and equipment was removed by workers other than the assigned Radiation Protection Technician.
This is an infraction.
