APPENDIX A NOTICE OF VIOLATION Southern California Edison Company Docket No. 50-362 San Onofre Unit 3 License No. NPF-15 During an NRC inspection conducted on July 3 through August 16, 1988, a violation of NRC requirements was identified. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

10 CFR Part 50, Appendix B, Criterion V states, in part, that "Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the.

circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

The SCE Topical Quality Assurance Manual, Chapter 5-A (Revision No. 7),

Paragraph 12, states, in part, that "Procedures shall be the basis for performance of actions...

covered by the quality assurance program."

Paragraph 6.13 of Maintenance Procedure 50123-1-1.7 (TCN 2-2 dated May 6, 1988), titled Maintenance Order Preparation, Use and Scheduling, requires compliance with work packages and procedures.

Blanket Maintenance Order (MO) 87112010, in the Precautions section, directed the worker to "Raise hoses or remove from pool to prevent siphoning when pump is secured."

Contrary to the above requirements, a temporary pump installed to transfer water from the reactor vessel lower cavity to the spent fuel pool was started for an operational test on June 22, 1988, and the pump discharge hose was not raised or removed from the pool when the pump was secured. As a result, and inadvertent siphon path was established between the spent fuel pool and the reactor vessel lower cavity.

This is a Severity Level IV violation (Supplement I) applicable to Unit

Pursuant to the provisions of 10 CFR 2.201, Southern California Edison Company is hereby required to submit a written statement or explanation to the U.S.

Nuclear Regulatory Commission, ATTN:

Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region V, and a copy to the NRC Senior Resident Inspector, San Onofre, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each 1 violation:

(1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
