Docket Nos. 50-206, 50-361, and 50-362 Revision 6.4 to the Site Emergency Plan San Onofre Nuclear Generating Station Units 1, 2, and 3 This letter transmits Revision 6.4 to the Emergency Plan for the San Onofre Nuclear Generating Station (SONGS). This revision (enclosed) replaces one of the letters of agreement in Appendix A of the Emergency Plan. This change maintains the plan current and does not decrease the effectiveness of the plan.
The medical facility which had operated as Samaritan Medical Center San Clemente now operates as Columbia San Clemente Hospital & Medical Center. A change in ownership precipitated the need to replace the existing letter of agreement between Southern California Edison (Edison) and Samaritan Medical Center San Clemente with a letter of agreement between Edison and Columbia San Clemente Hospital & Medical Center. The terms of the new agreement are the same as the previous agreement. The new letter of agreement provides medical services which are equivalent to the prior agreement.
The Emergency Plan revision will be effective on September 19, 1996.
Replacement pages for the SONGS Emergency Plan will be sent to the plan holders of record by separate letter.

1992 Renewal of reement for Medical Treatment Facilities This letter confirms the Agree ent between Samaritan Medical Center San Clemente (the "Hospi al") and Southern-California Edison Company ("SCE") concerhin provision of medical treatment facilities for the general medica treatment of SCE personnel from San Onofre Nuclear Generating tation (the "Station") and emergency medical treatment for oth individuals suffering from injuries, injuries complicated by ra 'ation contamination, or excessive radiation exposure as a cons quence of-'activity at the Station.
Staff training and certain e ipment to support the handling of radiation contaminated patie ts will be provided by SCE.
Confirmation of this Agreement is based on or current understanding that:
- 1.
The Hospital has the physical capacity, pe sonnel, medical equipment and resources to handle a radiati n contaminated patient as a result of an accident at the St tion and is accredited by the Joint Commission on Accredi ation of Healthcare Organizations (JCAHo).
- 2.
-The Hospital has or will grant temporary house p ivileges through the appropriate procedures as defined by e Medical Staff bylaws to those designated physicians who ha e their up-to-date credentials and license on file with the Hospital and have contracted with SCE to provide treatment or consultation to SCE personnel and other individuals o have been injured at.the Station or individuals from commun 'ties in vicinity of the Station who have been exposed to excessive radiation; or who have injuries complicated b radioactive contamination as a consequence of a radiologl al accident at the Station.

-2 January 9, 1992 San Clemente By executing acceptance of this letter, you confirm these under standings and agree to provide the following medical treatment services to SCE in support of the operation of the Station:
- 1.
Hospital care and treatment for SCE personnel or other individuals in support of Station operations who have been injured at the Station; or who have been exposed to excessive radiation; or who have injuries complicated by radioactive contamination as a consequence of a radiological accident at the Station.
- 2.
Hospital care and treatment for individuals from communities in the vicinity.of the Station injured as a result of activities at the Station, whose injuries may be complicated by radioactive contamination as a consequence of a radiological accident at the station.
SCE will be responsible for the payment of your reasonable fees and charges for any such services rendered at SCE's request.
This Agreement will remain in effect unless terminated by either party giving thirty (30) days advance written notice of termination to the other party.
Please signify your continued agreement to the provisions of this letter by executing the acceptance below and returning this letter to me in the enclosed self-addressed stamped envelope.
A copy of this letter agreement is also enclosed for your records.

("SCE") concerning provision of medical treatment facilities for the general medical treatment of SCE personnel from San Onofre Nuclear Generating Station (the "Station") and emergency medical treatment for other individuals suffering from injuries, injuries complicated by radiation contamination, or excessive radiation exposure as a consequence of activity at the Station.
