In accordance with the requirements of Title 10, Chapter 1, Code of Federal Regulations, Part 21, Enterprise Engine

Services, a Division of Cooper Industries, hereby notifies the Commission of a potential defect in a component of a DSR or DSRV Standby Diesel Generator System.

There exists a potential problem with intake and exhaust power valve springs which could result in engine non-availability.

Cooper Industries has supplied these parts as replacement components to the following sites:

Corry, Pennsylvania.

They are installed as a subcomponent of spare cylinder heads (4 springs per cylinder head) or provided as individual spare components by Cooper Industries.

The spring part number is 03-360-02-OM. They may also be installed on cylinder heads identified as part numbers 1A-5559, 1A-6240, 1A-6266, 1A 6446, 1A-7062 and 1A-7680.

In the course of inspecting existing stock of valve springs, we noted draw marks on some of the springs on the inside diameter. Destructive testing was performed on a sample lot to establish the depth of any draw marks and determine any potential for failure.

Our testing resulted in (1) piece with a seam while several others had draw marks deeper than allowed by specification.

All stock was removed from stores and returned to Associated Spring for further testing, whereupon (2) additional pieces were identified as having a seam.

The existence of a seam is not allowed by our specification nor are draw marks deeper than 0.002".

We are unable to determine how many, if any, valve springs with seams or draw-marks are in the field since previous inspections were based on sample lots.

Our level of concern,

however, is significantly reduced due to the fact that there are no reported failures of this spring in any nuclear or non nuclear installations.

It is important to note that the subject springs were received in 1986 with hundreds distributed to a variety of installations since that time; the commercial sites having accumulated thousands of hours in many cases.

The number of operating hours is significant as the fatigue failure resulting from such an indication should occur within approximately 740 hours0.00856 days <br />0.206 hours <br />0.00122 weeks <br />2.8157e-4 months <br /> (101 operating cycles).

It is our recommendation that the subject power springs be inspected on a periodic basis such as during the rotating refueling outage inspection of the cylinder heads.

Any spring displaying a draw mark on the I.D. that is visible to the naked eye (through the painted surface) should be removed, discarded and replaced with an acceptable spring.

It is our further recommendation that this periodic valve spring inspection should be included in the mechanical maintenance procedures and that the inspection should apply to all valve

springs, regardless of their original manufacturer.
