Dear Sir:

On the afternoon of March 30,

1990, we received by facsimile from Southern California Edison Company a copy of an Ederer letter dated April 12, 1985, which describes a problem found during test of an X SAM system in England.

Resolution of this problem resulted in revisions to drawings of a mechanical component called a "Slide."

These revisions occurred between March and August of 1985, and involved Ederer drawings B 15937, B-15938, B-16868, B-16936 and B-16937.

The April 12, 1985 letter also mentioned the X-SAM systems at Wisconsin Electric's Point Beach Plant and at Iowa electric's Duane Arnold Unit 1 plant.

These installations have Slides made prior to March 1985 and no retrofits have been made.

The X-SAM at Iowa Electric was field checked and tested in January 1985.

The Wisconsin installation has both main and auxiliary hoist X-SAM's, and they were field tested in August of 1985.

All had previously passed our in house check-out and tests.

It appears that the successful completion of factory and field tests (without incidence of this problem) for Iowa and Wisconsin led to the conclusion that no further action was warranted, and that the problem was not reportable under 10 CFR 21.

I do not agree with this decision,

however, and therefore have reported this defect by telephone to Mr. Greg Cwalina of the Vendor Inspection Branch on April 3,

1990.

On this date I also notified Wisconsin Electric, Mr.

Bruce Sasman, and Iowa Electric, Mr.

Bruce

Lacy, of this action.

The notifications to Wisconsin and Iowa included instruction to make no critical lifts until the X-SAM systems are retrofitted with Slides of new design.

I am enclosing a copy of the Ederer letter of April 12, 1985, along with our in-house Trouble Reports for the Wisconsin and Iowa installations. Attached to the Trouble Reports are Ederer's 10 CFR 21 Forms 1, 2 and 3 for each.
