Letter, Mr. Kenneth P. Baskin (SCE) to USNRC, dated August 12, 1987 As discussed with Mr. F. R. Huey (USNRC Senior Resident Inspector) on June 30, 1988, and in our telephone discussion of July 1, 1988, the referenced letter provided the SCE response to a Notice of Violation which included a commitment to perform verification and validation (V&V) on the Health Physics (HP) computer programs.
This action was taken because for lack of such V&V, an undiscovered computer program "bug" truncated the November 17, 1986, electronically received HP personnel exposure data, causing SCE be unaware until December 12, 1986, of a potential overexposure.
As we discussed, SCE initially performed a review of HP computer programs, and determined that approximately 20 HP programs should be subjected to independent V&V (until this ongoing V&V process is complete, supplementary means are being used to provide assurance of data quality).
The computer programs associated with the receipt of dosimetry results from the-dosimetry vendor, data transmission to the San Onofre Radiation Control (SRC) system and intermediate processing were V&V'd and subjected to controls by 88076036 880708 PDR ADOcK 05000206 PN

This work was completed on February.19,- 1987.
The HP Division revised site procedure S0123-VII-4.3.13 to require a physical comparison of electronically transmitted dosimetry data with the hard copy received from the dosimetry vendor. In addition, NIS developed and implemented S0123-XVII 10.13 to enhance the electronic data processing quality.assurance controls. This work was completed on April 14, 1987.
All HP computer applications were reviewed. Over 400 HP programs were catalogued and approximately 60 additional programs were identified as-quality affecting and were scheduled for independent V&V. To support this effort, NIS increased manning in support of HP from two to nine specialists.
Due to the significant effort required to complete all aspects of the HP V&V effort, we have been unable to meet our initial scheduled completion target date of July 1, 1988, which was provided in the referenced letter. As we discussed, our revised schedule is as follows:
