Y; This letter describes an incident that occurred at San Onofte cl Generating Station Unit 1. Submittal is in accordance with the reporting requirements stipulated in Section 6-.9.2.b of the Technical Specifications A

On Monday; January 24, 1977 the west residual heat removal pump was removed from service for the purpose of dismantling the pump for inspection replacement of nonconforming cooling water piping and replacement of the drive motor. At the conclusion of day shift, the scope of the remaining Work was correctly described.to the night supervising engineer by th,piping superintendent.

At 7:30 p.m., an operato making ins ections in the area observed pipe fitters and welders attempting to repair a leak in the cooling water return line from the east residual heat removal pump. They were directed to stop their efforts until a suitable course of action could be developed by the responsible si.ipervisors. The east residual heat removal pump was C i subsequently stopped and the cooling water valve closed for a few minutes A

while a temporary weld repair was made to the cooling water piping. The l pump was maintained operable through'the incident.

The subsequent 'investigation 'indicated that the night supervising "N

engineer did not properly communicate the scope of the wbrk to the c"aft personnel and they were under the impression that they were to' r0pla the ing water return lines on both residual het removal pumps.;A,'

-2 The following actions were taken relative to the incident:

-1. The clearance procedure used to identify and make safe the various components and systems to be worked on has been reviewed. It was con cluded that insufficient information was being made available to th workmen actually performing the work.

2.

The clearance procedure has been revised. When the work period exteds beyond one shift, the general foreman holding the clearance is fow required to hold a "tail-board" meeting with the foreman and all the other members of the oncoming crew. The general foreman will describe the work to be performed, the boundaries within which it is to be performed and any non-obvious hazards. He will then encourage response from the crew members indicating their awareness of the total job package.

With this new clearance procedure; we believe there will dt be a recurrence of an incident of this nature.
