Dear Sir:
This letter describes a reportable occurrance involving the Chemical and Volume Control System. Submittal is in accordance with the reporting requirement stipulated in Section 6.9.2.b(2) of the Technical Specifications.
On April 26, 1977, the north boric acid transfer pump failed to start.
Corrective action was initiated; a spare pump was installed and placed in service April 27, 1977. During the period that the north boric acid transfer pump was out of service, both the boric acid injection pump and the south boric acid transfer pump were available and the conditions of Technical Specification 3.2.B(2) were, therefore, satisfied.
Investigation revealed an accumulation of boric acid crystals in the failed pump. The pump lagging and heat tracing circuits were examined. Operating procedures were reviewed. No specific cause for the failure could be positively determined. However, to afford greater protection from potential development of isolated cold areas, additional lagging was added in the vicinity of the pump and adjacent piping.
