Dear Mr. O'Leary:
This report describes an incident experienced at San Onofre Nuclear Generating Station concerning No. 2 diesel generator.
On October 16, 1973, during the weekly test of No. 2 diesel generator, excessive smoke was observed coming from the exhaust.
The test was terminated and an investigation was conducted to determine the problem. This investigation revealed that a loose screw in the governor end fuel pump caused the No. 2 north cylinder to receive excess fuel thereby causing excessive exhaust smoke.
There were indications that the screw had been cross-threaded during its installation.
During the investigation, it was also noted that the south bank of cylinders was running colder than the north bank. Excessive wear was noted on the fuel rack forks of the generator end fuel pump.
This caused the south bank to run leaner and therefore colder than the north bank. This problem was not advanced to a degree where diesel performance was significantly affected. Both fuel pumps were over hauled and worn or damaged parts were replaced as necessary. The wear on the fuel rack forks was noted to some degree on the forks from both fuel pumps.
The fuel rack forks on the No. 1 diesel generator will be inspected to assure that a similar condition does not exist. An investigation to determine the cause of the excessive fuel rack fork wear is in progress and the results will be available for inspection at San Onofre.
Mr. John O'Leary November 20, 1973 In accordance with Technical Specification requirements, No. 1 diesel-generator remained operable during the time the No. 2 diesel generator was out of service.
