Dear Sir:
During spent fuel shipping operations currently in progress, a nondestructive magnetic particle inspection of the spent fuel shipping cask lifting devices was being conducted as part of a continuing lifting device inspection program.
On May 3, 1976, in the process of this inspection program, a defect was discovered in welds on the rectangular coupling device which is positioned between the extension shaft and the sister hooI: adapter as shown in Figure I.
Figure 11 details the rectangular coupling device with pertinent dimensions.
As shown in Figure II, the defective welds secure block A to sides 1 and 2 to foir the rectangular coupling device, Based on the above, the existing rectangular coupling device has been determined unsuitable for further service in its present condition.
Appropriate corrective measures will be taken before spent fuel shipments are resumed.
